I hereby give The 8th Count Performing Arts Company, LLC permission for the public display of pictures and/or video of my child for possible advertisement purposes, including, but not limited to brochures, facebook, instagram, website, as well as other 8th Count publications.

I hereby release and hold harmless The 8th Count Performing Arts Company, LLC from any reasonable expectation of privacy or confidentiality for the child and children associated with the images specified above. Further, I attest that I am the parent or legal guardian of this child or children and that I have full authority to consent and authorize The 8th Count Performing Arts Company, LLC to use their likenesses.

I further acknowledge that participation is voluntary and that neither I, the minor child, or minor children will receive financial compensation of any type associated with the taking or publication of these photographs or participation in company marketing materials or other publications for The 8th Count Performing Arts Company, LLC. I acknowledge and agree that publication of said photos confers no rights of ownership or royalties whatsoever.
