ADA General Waiver

Thank you for joining us at ADA! Please read and accept the following waiver:
PAYMENTS: We require a payment method to be saved on file, but you may also pay with cash or check prior to the due date to avoid autopay. Should it become necessary to discontinue classes for any reason, YOU MUST notify the office in writing two weeks before a withdrawal can be permitted using the “Student Withdrawal Form” located on the website. This is required or ADA retains the right to keep the student’s enrollment status active and charge accordingly. You are financially responsible for tuition payments up to the time of such action. All payments are non-refundable. Credit, if approved, can be shared with immediate family members only. A Registration Fee is due anytime one re-starts within the September-June season.
CHECKS: Any returned checks will be charged an additional $30 payment.
CREDIT CARD/ACH: A 2.7% Convenience Fee will be added to all payments made by credit card and 0.8% convenience fee added to payments from ACH. This fee will be due at the time of online payment and is non-refundable.This surcharge is not greater than our cost of acceptance. Cash and check payments do not incur a convenience fee.
INSURANCE: American Dance Academy does not carry medical insurance for its students. It is required that all students be covered by their own family insurance policies and, if an injury occurs, it is understood that the student's own policy is the only source of reimbursement. Parents must understand and agree that by participating in any dance class, workshop, rehearsal, or performance, there is a possibility of physical injury. Parents voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to my child or self during any of American Dance Academy classes, lessons, rehearsals, performances, or activities.
OTHER
• No gum chewing in class.
• Long hair must be tied back and secured for class.
• ADA is not responsible for unsupervised children who are not in a class. Please drop off and pick up your children on time. In case of an emergency, call the front desk at 302-234-9900.
• Dancers and parents are expected to conduct themselves in a respectful manner. Unkind words about other students, parents, or teachers are unacceptable and will not be tolerated.
*ADA uses photos of classes for promotional purposes. If you are not in agreement, please send a separate email so we can keep that on file.
*Water breaks are permitted in between classes. Drinks are not permitted in our dance rooms.
