Payment Agreements
I UNDERSTAND THAT ALL TUITION'S & COSTUME FEES ARE DUE NO LATER THAN THE 10TH OF EACH MONTH AND PAYMENTS ARE EXPECTED REGARDLESS OF STUDENT ATTENDANCE. (STUDENTS CANNOT SKIP MONTHS.) REPLACEMENT CLASSES ARE AVAILABLE FOR MISSED CLASSES. TUITION'S, COSTUME DEPOSITS, COMPETITION FEES, REGISTRATION FEES, AND ALL OTHER FEES ARE NON-REFUNDABLE.I agree to pay any late fees that occur due to payments not received on time.

Liability waiver
I, the undersigned, understand and agree that I shall not hold Branches of Dance, its staff, and/or volunteers responsible in any way for any injury, accident, death, or virus, which might occur during classes or any function of said Branches of Dance. I assume all responsibility for any damages my child may cause on the Branches of Dance premises. The above statements also apply if I, the undersigned, am on the premises of Branches of Dance.

Drop Waiver
I, the undersigned, understand and agree that by enrolling my child in Branches of Dance, I am committing to pay the full tuition fee for the entire duration of the class. In the event that my child drops out of the class after 3 months, I will be responsible for paying a late season drop fee of $100 per class dropped. This fee is to cover the cost of choreography and items that have been ordered for my child.
I understand that after 3 months in the class, my child's position cannot be replaced and that their absence may impact the entire group's performance. Therefore, I agree to uphold my commitment to the class and understand that failure to do so will result in a $100 drop fee per class.
By signing below, I acknowledge that I have read and understand the terms of this waiver and voluntarily agree to its content.

Photo Release
I give my consent for images (photographs, video) of my dependent(s) to be taken and used to document the activities of Branches of Dance. I grant Branches of Dance permission to use the images for educational and promotional purposes. I understand that if I do not want images of my dependent to be used, I will indicate this in writing and the signed letter will be attached to this document. I have read and understand all the policies of Branches of Dance and understand that they must be followed in order to participate in any of Branches of Dance programs.
