Agreement for Participation

Release of Liability
I understand that dance classes may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance program involve some degree of risk of strain or bodily injury. I HEREBY WAIVE, RELEASE, AND DISCHARGE East Coast Dance Company LLC and all divisions thereof of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and/or instruction of dance classes, camps, intensives, private instruction, choreography or any activity I may participate in at the studio. I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: East Coast Dance Company LLC and/or their directors, officers, managers, employees, volunteers, representatives, and agents, the activity or event holders, activity or event sponsors, activity or event volunteers; As the legal parent or guardian, I release and hold harmless East Coast Dance Company LLC, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant while in or upon the premises or any premises under the control and supervision of East Coast Dance Company LLC, its owners and operators or in route to or from any of said premises.

Video & Photo Waiver
I give East Coast Dance Company LLC my permission for the public display of pictures and/or video of my child for possible advertisement purposes (i.e. brochures, newspaper ads, website, Facebook page, etc.) Photos and videos may be used for educational and informational purposes regarding the programs at East Coast Dance Company LLC.

Handbook Notification
I understand the ECDC handbook is online and agree to adhere to all the content stated therein including studio policies, tuition and payment information, attendance policies, dress code, traffic pattern, visitor weeks, and calendar. I agree to be responsible for reading studio correspondence and respecting deadlines.
