Insurance/Liability Waiver:

I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is the possibility of physical injury. I agree, therefore, to assume all risks of any such injury to myself, or my minor child, which might occur during any and all Visions In Motion classes, rehearsals, or outside related activities such as parades, recitals or performances. I exempt, release and indemnify Visions In Motion Dance, Inc. (“Visions”), its owners, instructors, agents, advisors, employees, guest artists, volunteers and faculty members from any and all liability claims, demands or causes of action whatsoever from any damage, loss or injury to me (or my minor child) or my (or my minor child’s) personal property which may arise out of or in connection with participation in any classes or activities conducted by Visions, whether such loss, damage or injury results from the negligence, passive or active, of Visions, its instructors, agents, employees, guest artists, volunteers or faculty members or from some other cause.

Model Release

I grant to Visions In Motion Dance, Inc. (“Visions”) the sole absolute, unrestricted and irrevocable ownership of any photographs or videos taken during Visions classes and/or performances in which I may appear; to use, re-use, for any purpose whatsoever including but not limited to documentation, education, promotion of goods and services, art, editorial, advertising, without restriction as to alteration. I hereby release and discharge Visions and any and all photographers/videographers hired by Visions or working voluntarily on behalf of Visions from claims for libel or invasion of privacy or for payment. This authorization and release shall also inure to the benefit of the heirs, legal representatives, licensees, and assigns of Visions and any photographers hired by Vision. This release shall be binding upon me and my heirs, legal representatives, and assigns. If student is under 18, I release as stipulated above on behalf of my child.
