Liability Waiver
I realize that participation in dance classes and activities could involve some possible personal injury. I recognize that dance classes require physical exertion that may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved. I understand that it is my responsibility to consult with a physician prior to and regarding my child's participation in dance classes. I represent and warrant that my child is physically fit and has no medical condition that would prevent them from full participation in dance classes. Despite precautions, accidents and injuries may occur. By signing this release form, I agree my child will obey the class and facility rules and take full responsibility for their behavior in addition to any damage they may cause to the facilities utilized by Studio 8 Dance LLC. Furthermore, I assume all risks related to the use of any and all space used by Studio 8 Dance LLC. I agree to release and hold harmless Studio 8 Dance LLC including its teachers, dancers, staff members and facilities used by both entities from any cause of action, claims or demands now and in the future. I will not hold Studio 8 Dance LLC liable for any personal injury or any personal property damage which may occur on the premises before, during or after classes. **I also understand that participation in any in person classes may result in possible exposure to COVID-19 without the studio's knowledge. I agree to hold Studio 8 Dance LLC harmless should me or my child, or anyone who enters the premises, contract COVID-19.**
Photography Release
I grant to Studio 8 Dance LLC, its representatives and employees the right to take photographs of me or my child. I authorize Studio 8 Dance LLC its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Studio 8 Dance LLC may use such photographs of me or my child with or without names and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content.
Financial Responsibility
I understand the financial responsibility I will incur by enrolling my child in dance class. I understand tuition bills are due by the 10th of each month, and any outstanding tuition will automatically incur a $15 late fee. I also understand that any monies paid in conjunction with a 3rd party vendor company (costumes, competitions, merchandise, etc) are completely non-refundable.
