1. I undertake to pay fees by the termly due date. 2. I will give a full terms written notice if I wish to discontinue classes.
By registering online I hereby accept that my child/I myself (shall be refered to as dancer) will take part in the dance lessons at Christian Dance Academy. I fully understand and accept that all activities, use of equipment and facilities will be undertaken at the dancer's own risk and I undertake on behalf of myself, my executors, my wife/husband aforesaid, to indemnify and keep indemnified the Principal and the Staff of Christian Dance Academy against any or all claims and/or actions whatsoever that may arise in connection with any loss of, or damage to, the property or injury to the person of the dancer aforesaid in the course of such activities either at or on the studio (13 Utrecht Ave, Bonnie Doone) and / or NG OL Noord church hall (cnr of Devereux Ave and Chamberlain Road, Vincent) property. I am aware of the serious health risks associated with the rapid spread of the Coronavirus (COVID-19) worldwide. I take note that whilst the school has taken all reasonable and regulatory steps to ensure the health and safety of all pupils and staff ; in agreeing to this declaration I acknowledge that I have /my child has entered the school premises at my /his/her own risk and indemnify the school from any liability in the event of my/my child contracting Covid-19 or any related illness as a result of my / my child having entered the Christian Dance Academy premises .
Once accepted, I undertake: 1. To pay fees by the due dates stipulated on the fees document. 2. Give a full term's written notice if I wish to discontinue classes.
~ PLEASE NOTE: After Ballet Exams in June / July, students may move up a grade in the following term. In some cases this means an increase in the length / frequency of lessons with an increase in fees as well. ~ PAYMENTS MAY BE DIVIDED INTO 2 PER TERM, PLEASE ADVISE KATI IF YOU PLAN TO DO SO. Please keep an open dialogue with us if there is a problem keeping up to date. ~ NOTICE POLICY: If wanting to discontinue classes, a full term's notice needs to be given in writing by email to info@christiandanceacademy.co.za ~ OUTSTANDING FEES: Outstanding fees at the end of a term will render a student ineligible to return to class.
