I acknowledge that myself or my child’s participation in activities at Studio VIE are voluntary “at your own risk†participation and will not hold Studio VIE, property owners, Wilhoit Properties, CLI Studios, designated supervisors or chaperones, or any instructor of classes, workshops, or events in which I participate liable for injuries or accidents sustained during said activities.
I understand that Studio VIE may use my face, name or likeness via video, social media, print ad, or other forms of advertisement for the purpose of promotion of classes, activities or group events.
I fully understand that Studio VIE staff members are not physicians or medical practitioners. With that fact in mind, I hereby release Studio VIE staff to render temporary first aid to myself or my children in the event of any injuries or illness. I understand that I will be contacted in case of serious injury, and acknowledge that Studio VIE staff may seek help from medical practitioners, including calling of an ambulance to a hospital, if deemed necessary.
I ACKNOWLEDGE AND UNDERSTAND THAT DANCE, GYMNASTICS, TUMBLING, AERIAL, AND FITNESS ACTIVITIES ARE INHERENTLY HAZARDOUS and involve both known and unanticipated risks which could result in damage or destruction of property and physical or emotional injury, including paralysis or death, of myself or of other persons . I FURTHER ACKNOWLEDGE AND UNDERSTAND THAT I am personally responsible for mine or my family’s conduct while participating in the Activity and that any failure on my part or my child’s part to follow instructions, whether willful, reckless or negligent, shall be my sole responsibility. I further understand and agree that I am fully, and solely responsible for myself and my child’s safety, and assume the full risk of any physical injury including death; emotional injury; and other damages that may arise from failure to adhere to the general safety instructions and/or guidelines for safe execution of skills of Studio VIE staff, employees or management.
This liability waiver holds true for any participation in activities at 1515, 1517, or 1519 East Lark, or any workshop, competition, performance, or event off the premises under the care and supervision of Studio VIE employees or designated chaperones. This includes participation in events and/or travel to and from events.
Should injury occur and limit myself or my child’s participation in events in which payment has already been rendered (including competitions or costume fees) I understand that I will not receive a refund for said materials unless a refund is provided by the original company to Studio VIE. I understand that I am not liable for payments incurred for classes unless my child participates in said classes and receives instruction, no matter how limited.
With the above in mind, having read this agreement in full and being fully aware of the risks and possibility of injuries, I consent to have myself, my child or children participate in programs offered by Studio VIE. I waive and release all rights and claims for damages or liability that myself, my child, or my representatives/family may have against Studio VIE and/or its representatives and affiliates.
