I agree that I am over 18 years of age and that I am the parent/guardian of a child/ren who will receive dance education services from Dance Dimensions. I agree that the total amount due for monthly lessons is due and payable on the 1st of every month. If I fail to pay as agreed above when due, Dance Dimensions will charge a $10.00 late fee for all payments received after the 15th of each month. I will also pay an additional fee for each month there after. I agree to pay for costumes, alterations (if needed) and accessories. I understand that if monthly lessons and costumes= fees are not paid in full by June 1st my child/ren may not be allowed to perform in the annual recital. I understand if my child/ren discontinues class after January 1st, I will be responsible to pay for any costumes that have been ordered. In the event my account is turned over to an attorney for collections, I agree to pay reasonable attorney fees. There will be a fee of $20.00 for all insufficient funds. This contract valid for the 2020/2021 dance year.
