Liability Release Waiver

1. Agreement to follow directions. I agree to observe and obey all posted rules and warning, and further agree to follow any oral instructions or directions given by any employee of Statesboro Elite Dance, LLC.

2. Assumption of the risks and release. I recognize that there are certain inherent risks associated with the above described activity and I assume full responsibility for personal injury to myself (and if applicable) my family members, and further release and discharge Statesboro Elite Dance, LLC for injury, loss or damage arising out of my or my family’s use of presence upon the facilities of Statesboro Elite Dance, LLC, whether caused by the fault of myself, my family, Statesboro Elite Dance, LLC or other third parties.

3. Indemnification. I agree to indemnify and defend Statesboro Elite Dance, LLC against all claims, causes of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my or my family’s use of or presence upon the facilities of Statesboro Elite Dance, LLC.

4. Photography, Video, Advertisements. I give Statesboro Elite Dance, LLC permission to use photos and videos of myself/my child/the minor child for whom I am legal guardian for promotional purposes. including, but not limited to: in the dance studio, on the Statesboro Elite Dance website, or in the community.

5. Fees. I agree to pay for all damages to the facilities of Statesboro Elite Dance, LLC caused by any negligent, reckless, or willful actions by me or my family.

6. Applicable Law. Any legal or equitable claim that may arise from participation in the above shall be resolved under Statesboro Elite Dance, LLC law.

7. No Duress. I agree and acknowledge that I am under no pressure or duress to sign this Agreement and that I have been given a reasonable opportunity to review it before signing. I further agree and acknowledge that I am free to have my own legal counsel review this Agreement if I so desire. I further agree and acknowledge that Statesboro Elite Dance, LLC has offered to refund any fees I have paid to use its facilities if I choose not to sign this Agreement.

8. Dispute Resolution. The parties will attempt to resolve any dispute arising out of or relating to this Agreement through friendly negotiations amongst the parties. If the matter is not resolved by negotiation, the parties will resolve the dispute using the below Alternative Dispute Resolution (ADR) procedure. Any controversies or disputes arising out of or relating to this Agreement will be submitted to mediation in accordance with any statutory rules of mediation. If mediation does not successfully resolve the dispute, then the parties may proceed to seek an alternative form of resolution in accordance with any other rights and remedies afforded to them by law.

Statesboro Elite Dance Studio
Illness Policy

While we would love to have every single dancer healthy every day of the year, unfortunately sometimes they fall ill. We strive to limit the spread of communicable disease in our studio and are committed to implementing policies that balance and respect the needs of children, families, and staff in these circumstances.
Staying home from the studio and class is sometimes necessary to reduce the transmission of serious illnesses. Illness happens, and our goal is to keep our whole community of dancers safe and healthy. To reduce the spread of severe illness, we ask parents to help us by honoring our illness policy.

All illnesses, and the specific policies around them, are different, but we ask that parents keep their children home if they exhibit the following symptoms:
• Fever of greater than 100°F without use of fever-reducing medication, especially with behavior- symptoms that prevent a child from participating in dance activities.
• Diarrhea or stool frequency
• Vomiting with fever or vomiting more than two times in 24 hours
• Rash with fever or behavioral changes (unless a physician has determined it is not a communicable disease)
• Purulent conjunctivitis (defined as pink or red conjunctiva with white or yellow eye discharge) until on antibiotics for 24 hours.
• Strep throat (or other streptococcal infection) until 24 hours after treatment has been started.
• Head lice until after first treatment
• Chickenpox, until all lesions have dried or crusted (usually 6 days after onset of rash).


Children may return when:
• They are free of fever, vomiting, and diarrhea for a full 24 hours.
• Re-admission after diarrhea can occur when children do not have toileting accidents
• They have been treated with an antibiotic for a full 24 hours.
• They are able to participate comfortably in all usual dance activities
• They are free of open, oozing skin conditions and drooling unless
o the child’s medical provider signs a note stating that the child's condition is not contagious, and
o the involved areas can be covered by a bandage without seepage or drainage through the bandage.

Rules & Regulations

• No parents or friends allowed in class except during designated viewing days.
• No food and/or chewing gum allowed in the studios. No open containers, water and/or sports drinks must have a cap. Water bottles are strongly encouraged for short water breaks.
• Statesboro Elite Dance employees or instructors reserve the right to dismiss any dancer from class whose attitudes are unsatisfactory. We promote all dancers to treat each other with respect, support and encouragement at all times.
• All classes will begin on time. Please be ready and on time for class. Hair, clothing, and shoes are on and ready before class begins.
• The studio will be opened 15 minutes prior to the first class of the day. Students under the age of 8 are not to be left unattended before their class begins.
• Please do not enter the dance studios before your designated class time out of respect for the class in session.
