ENROLMENT TERMS AND CONDITIONS:
" I understand students are to be dropped off and collected within 10 min prior to and at the conclusion of class times unless supervised by a parent and student must comply with dress and behaviour etiquette.
" I acknowledge tuition fees are payable per term, are to be paid in full by the 2nd week of term and the invoice is emailed by the accounts department. I agree late fees will be payable in the event of an overdue account and my details may be passed onto a collection agency upon failure to pay tuition fees. I understand other fees may be payable throughout the year: i.e. costumes (including concert costumes), exam fees, workshops, seminars etc.
" I understand that if I leave the studio, or my child moves up an age group, I am required to make available for sale all eisteddfod costumes if need be.
" I understand that if at any time my child discontinues dance tuition at PPDC, that I am required to give 30 days' notice in writing to Janine Tirant, otherwise Term fees are payable - (see fees terms and conditions on website)
" I understand that CCTV will monitor premises/property at PPDC for damage and security purposes. I acknowledge that if my child intentionally defaces walls/property at PPDC I accept that repair/replacement costs are payable.
" Signing this enrolment form is my acknowledgement and acceptance of all of the above terms and conditions and I have read and understood Performance Plus Dance Centre fees terms and conditions.
