I release Adagio Dance Studio, it’s instructors, independent contractors, and all other associates from liability for harm, theft, or injury that may be suffered by me and/or members of my family traveling to or from or during participation in activities and programs sponsored by Adagio Dance Studio. I hereby acknowledge that I am voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in a dance class or other dance related activities. I acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntary assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program participants and their families. I release Adagio Dance Studio, its instructors, independent contractors and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses. I understand and agree to Adagio Dance Studio policies and procedures. I understand that my student must have this release form signed before attending class. I understand and have read all the studio and policy information.

By electronically accepting, I am stating that of my own accord I have read, understood, and agree to all the terms and conditions in this COVID-19 Release Waiver.

Authorization and consent of parent(s) or legal guardian(s) I do hereby state I have legal custody of the aforementioned Minor/Student.

I grant my authorization and consent for Adagio Dance Studio (hereafter "Designated Adult") to administer general first aid treatment for any minor injuries or illnesses experienced by the Minor. If the injury or illness is life threatening or in need of emergency treatment, I authorize the Designated Adult to summon any and all professional emergency personnel to attend, transport, and treat the minor and to issue consent for any x-ray, anesthetic, blood transfusion, medication, or other medical diagnosis, treatment, or hospital care deemed advisable by, and to be rendered under the general supervision of, any licensed physician, surgeon, dentist, hospital, or other medical professional or institution duly licensed to practice in the state in which such treatment is to occur. I agree to assume financial responsibility for all expenses of such care. It is understood that this authorization is given in advance of any such medical treatment, but is given to provide authority and power of the part of the Designated Adult in the exercise of his or her best judgment upon the advice of any such medical or emergency personnel.
This authorization is effective immediately and through the duration of enrollment at Adagio Dance Studio.

By electronically accepting, I am stating that of my own accord I have read, understood, and agree to all the terms and conditions in this Emergency Medical Treatment Consent.

Authorization and consent of Parent(s) of Legal Guardian(s) I do hereby state that I have legal custody of the aforementioned Minor/Child.
I give Adagio Dance Studio, permission to use my child's, likeness, image, voice, and/or appearance as such may be embodied in any pictures, photos, video recordings, audiotapes, digital images, and the like, taken or made on behalf of Adagio Dance Studio activities. I agree that Adagio Dance Studio have complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with Adagio Dance Studio. These uses include, but are not limited to illustrations, bulletins, exhibitions, videotapes, reprints, reproductions, publications, advertisements, and any promotional or educational materials in any medium now known or later developed, including the Internet. I acknowledge that I will not receive any compensation, etc for the use of such pictures, etc., and hereby release Adagio Dance Studio from any and all claims which arise out of or are in any way connected with such use.

By electronically accepting, I am stating that of my own accord I have read, understood, and agree to all the terms and conditions in this Photograph, Video & Publicity Release.

Authorization and consent of parent(s) or legal guardian(s) I do hereby state I have legal custody of the aforementioned Minor/Student.

I hereby agree to indemnify, defend, and hold harmless Adagio Dance Studio, its officers, and employees, agents and volunteers, form against any and all claims, damages, liability, expenses, and judgments, including attorney's fees, in any way arising from my (or my child's) participation in the program for which I am registering my him or her. This includes, but is not limited to, any claims damages, liability, expenses, and judgments, including attorney's fees, in any way arising from any medical treatment received by me (or my child) made in connection with my (or my child's) participation in the program for which I am registering him or her. I understand and am familiar with the nature of the activities entailed by my (or my child's) participation in the program for which I am registering him or her. I understand that this program can be dangerous to me (or my child) and I understand that by participating, I (or my child) participate(s) with full knowledge of such risk of danger.
