You must be over 18 to register for an account.

I hereby waive all claims against Academy of Dance, Music, & Theatre and employees arising from injuries to said family members, which said injuries are sustained while in the supervision of all employees during the participation of activities including dance, music, or theatre classes, rehearsals, recitals, camps, and/or birthday parties, on the premises of the school or such area designated by the school for student related purposes. I understand that that the Academy may photograph participants enrolled in programs and these photos may be used in future publicity. ADMT uses a variety of communication methods: Email, SMS, Facebook, Twitter, Instagram, Signs & Students.

I, the undersigned, understand and acknowledge that dance, music, and theatre classes at Academy of Dance, Music & Theatre ("the Studio") may involve some risk of personal injury or death. I assume full responsibility for any and all injuries and damages that may occur to me, my child or anyone else attending the classes on the premises of the Studio, or in connection with the classes offered by the Studio. I release the Studio, its directors, officers, instructors, employees, agents, and volunteers from any and all liability and claims for any injuries, damages, or loss that may arise from my or my child's participation in the classes or use of the Studio's facilities.

I understand that the Studio may take photographs or videos during classes for social media and marketing purposes. I grant the Studio permission to use these photographs or videos for these purposes and waive any right to compensation for their use, both during and after our enrollment at the Studio.

I acknowledge that I am financially responsible for the classes I am registering for and agree to pay for all classes by the designated due date. A late fee of $25 will be applied to all declined/late payments. I understand that opting into the auto-pay system is required for all students, including those who pay in full at the beginning of the term.

I understand that no refunds will be given for classes missed or dropped, but that paid funds can be applied as a credit towards future classes. I understand that I am able to make up a missed class in an alternative class within 2 weeks of the date that I/my child missed.

I accept the transaction fees (1% for ACH and 3.5% + 30 cents for Credit Cards) applied to all charges.

I understand and agree that my child may not participate in dance classes if they are not wearing athletic/appropriate dance attire with their hair tied away from their face.

I am in accordance that if inclement weather occurs, the studio will offer options for make-up classes and it is my responsibility to attend the make-up lesson and that no refunds will be administered.

I understand that private lessons have a no-call, no-show, no-rescheduling policy and that it is my responsibility to cancel a lesson no less than 24 hours in advance to be guaranteed a rescheduled time slot.

I accept that ADMT will reach out to me in a variety of methods such as phone calls, SMS (text), emails, written notices, etc. I understand that it is my choice to unsubscribe from any SMS/Email lists, but the Studio is not responsible for any information I may miss due to that choice.

By signing this waiver, I acknowledge that I have read and fully understand its terms and conditions and that I voluntarily agree to be bound by them.
