NOTICE: DAY, TIMES & FEE May Change at any time due to COVID19 State Regulations. We will do our best to accommodate changes such as virtual dance classes and adjusted fees if the program has to close due to COVID19 issues.
ATTENTION: If you or your child are showing symptoms of any flu, respiratory illness, head cold or symptoms of the disease or temperature above 99.8 please keep your child home to prevent the spread of any illness to others.
Here at South Fulton Studios our top priority is always the health and safety of all children, students, their families, and staff. CORONAVIRUS DISEASE 2019 (COVID-19) UPDATE
Here at South Fulton Studios our top priority is always the health and safety of all children, their families, and the child care workforce. As we continue to respond to COVID-19 Coronavirus in Georgia, we will continue to update our families. We appreciate your patience and understanding as we work through the many questions and issues related to this unprecedented public health emergency.

SFS Camp Programs must restrict families’ access to parts of the program.
Specifically, access must be limited to the front door of the facility
Health Screenings upon arrival (temperature check).
Instructors & Staff will provide temperature checks for children and staff on arrival.
All visitors, staff, and children in facility shall be screened prior to entrance and shall be excluded if they have a fever above 100.4 degrees Fahrenheit or exhibit respiratory symptoms, including cough or shortness of breath.
Staff conducting daily health screenings should use the appropriate PPE.
There will be modified sign-in/sign-out procedures.
Instructors must sign children in and out of the facility for the families if they do not use a computer/keypad system.
key pads to check-in/check-out or tablet must be disinfected after each use.
If parents are required to sign any documents they must sanitize their hands & use their own pen.
Students will eat snack in their classroom
Field Trips Currently Suspended Until further Notice
Sanitize and disinfect. In addition to the rigorous hygiene, sanitation, and disinfection licensing rules child care providers adhere to in their programs, special attention will be paid to the following after every use and every hour.
Surfaces and objects that are frequently touched, especially toys and games must be cleaned. This includes cleaning objects/surfaces not ordinarily cleaned daily such as doorknobs, light switches, classroom sink handles, countertops, nap pads, toilets, desks, chairs, cubbies, and playground structures & equipment.
ALL items that cannot be cleaned and sanitized will not be used.
And remember best practices:
Stay home if you are sick!
Wash your hands often with soap and warm water for at least 20 seconds. If soap and water are not available, use an alcohol-based hand sanitizer.
Avoid touching your eyes, nose, and mouth with unwashed hands.
Cover your cough or sneeze with a tissue, then dispose of the tissue in the trash.
Avoid close contact with people who are sick.
Get a flu shot!
Clean and disinfect frequently touched objects and surfaces.

STUDENT MEDICAL RELEASE
STUDIO: South Fulton Studios Performing Arts School
I certify that I or Student registering is physically capable and able to fulfill requirements to be an active participant of the dance/karate curriculum of South Fulton Studios Performing Arts School. I understand this form legally releases all obligations and responsibilities for medical treatment of student, in case of illness or injury during any dance related activity when either parent or emergency contact can not be reached. If there is any physical or medical reason why she or he should not participate fully, South Fulton Studios Performing Arts School requires a doctor’s release. Furthermore, South Fulton Studios and its staff are not liable for any injury during classes, rehearsals, performances, or any other event affiliated with South Fulton Studios Performing Arts School.
CLASS ACTIVITY POLICY WAIVER
I understand and acknowledge the risks of injury that are inherent in any program involving physical activity and/or transportation, and as the parent/guardian of the child registered herein, I hereby waive and release any and all rights and claims for damages I may have against South Fulton Studios, facility directors, instructors, employees or owners and any sponsors and their respective representatives for any loss of property or for any and all injuries, sickness or death that may be sustained in connection with participation in this program. Also, I have no objection to photographs/videos taken at the center being used in advertising or other media efforts. I do hereby give my consent to medical care, emergency or otherwise, inclusive of any necessary transportation, in the event of injury to my child/ward.
Please Note South Fulton Studios Policies (Please Read the following)
__Student must have appropriate dancewear for each class registered
__It is the parent/garden’s responsibility to see that his/her child is picked up immediately after class.
__You must register your child each session in order to guarantee your child’s spot in class.
__No jewelry to wear during class, especially hoop earrings and necklaces. Hair must be pulled back in a ponytail. __Classes with five or less children after the first week will be closed. You will be notified when registration is higher for reopening of class.
_Our emphasis and energies are ALWAYS on safety first, but please remember that dance, gymnastics, cheerleading, and karate are potentially dangerous sports. Any activity involving motion, rotation, or height creates the possibility of serious accidental injury.
____Because your child’s safety is important to us, it is imperative that you be on time to class. The first 10-15 min of class are warm-ups. If your child is 10 minutes late (or more), they will not be allowed to participate. Your tuition pays for your child’s spot in class. Because of this, payment is to paid even if child is absent from class. We will allow one make up class per session.
___SFS always offers a free trial class to make sure you & your child are as excited about our program as we hope they will be. Once your child register and fills a spot in class, no refunds will be given, with the exception of moving or a medical or family emergency. All refunds must have the approval of the owners.
____With your permission your child may be photographed, videotaped, or interviewed for stories/articles promoting SFS These stories/articles may appear on Facebook, Instagram, Ticktoc, Youtube, newspapers or on television news shows & more. __ I understand these important Policies of South Fulton Studios and agree to abide by them.
I certify that I or my child has had a physical exam in the past year and is in good physical health. I have been informed that South Fulton Studios does not carry a supplemental insurance policy and that it is my responsibility to carry adequate health insurance and pay any additional medical fees not covered by my health insurance. Any activity involving motion or height creates the possibility of a serious injury. I agree that South Fulton Studios may approve transport in case of an emergency.
ALLERGIES
To Avoid Allergies Parent must indicate if student has any allergies at the time of registration. Management will notify staff with updated allergy list that is posted for class. Staff also is trained to use Allergy Action Plans.
BIRTHDAYS
Please notify the facility Staff at least one week prior to the date so that a celebration can be planned into the schedule. Parents may provide a pre packaged snack for their child's group for the celebration.
CELL PHONES
We understand that parents want to stay connected with their children throughout the day and cell phones come in very handy. However, we discourage usage of the phone during our sessions except for letting parents know that they have arrived at the facility or in cases of emergency. If a parent needs to contact their child after arrival to the facility our business phone is readily available.
CENTER ACTIVITY SCHEDULE
Individual activities determine the number of children that can be accommodated during each activity period. Some activities are repeated from one week to the next so that children may experience a variety of interests.
CLOTHING & PERSONAL BELONGINGS
Children should wear clothing suitable for an active day at the studio. Parents are advised to MARK their child's first and last names on all clothing, bags, towels and lunch boxes. Parents are encouraged to make sure their child comes home with his/her proper bag and lunch box at the end of each day. Staff cannot be held responsible for children's personal belongings. While every effort will be made to locate lost items, the facility is not responsible for replacing them. Items left will be placed in a lost and found box. All items not claimed by the end of each session will be donated to a worthy cause.
COMMUNICATION
Notices may be sent home with your child to inform you about special events and/or other information that families need to know regarding the center. Please make sure to check your email, Dance Studio Pro Account and with your child or instructor about these notices. We will use texting as an additional avenue of communication for those that sign up for texting.
DISABILITIES
In order for South Fulton Studios to provide the best and safest experience as possible, we regret that we cannot accept children with disabilities. We cannot provide aids for disabled children or administer medication throughout the day. We regret any inconvenience this may cause.
DISCIPLINE
Good behavior will be elicited in a positive and kind way. Children will be given understandable guidelines for their behavior so that they develop internal control of their actions. The staff will work cooperatively with parents, keeping them informed of behavior issues and methods used to teach and guide the child towards socially acceptable behavior. Behavior issues that cannot be resolved may result in dismissal of the child. No refund will be given if this occurs.
HAZARDOUS MATERIALS- When not in use, all hazardous materials should be stored in the original container in a locked cabinet or room that has a child-resistant lock and is not accessible to children. Chemicals should be stored separately from food and medications. All hazardous materials should be used according to the manufacturer’s instructions on the label. Pesticides and other chemicals should not be used when children are present. Chemicals used to treat lawns should be restricted to chemicals that are approved for use in areas where children will be present. All hazardous material are disposed away from facility and placed in outside dumpster
Tuition, Payments, Withdrawal, Behavior & Make Ups
TUITION & FEES
1.Summer Dance Intensive Camp Tuition or Elite Dance Competition is due every Monday of each week. 2. Week Day Classes & Evening Classes Tuition Due Every 1st of the Month. We accept DEBIT cards, VISA/MasterCard, and Online payment. No personal checks will be accepted in order to keep operational costs down. An application with required documentation along with tuition fees must be summated prior to attending South Fulton Studios LLC . This will assist us in having the required ratio of adults with students.

Payments received after payment is due a late fee of $25.00 per week will be added.
Field trips are to be additional unless stated on permission slip.
REFUND POLICIES
No prorating or make-ups for partial attendance. No refunds or credits for withdrawals or cancellations.
WITHDRAWAL – if withdrawal becomes necessary, it is the responsibility of the parent/guardian to bring this fact to the staff’s attention in email
BEHAVIORAL RULES – all students are advised of the rules and regulations during the orientation and thereafter each session. Failure to abide by the rules, general misconduct and/or possession of illegal substance will be grounds for immediate suspension or dismissal from the facility without refund. Parents or Student will be held responsible for and will be billed for any damage caused by their child to the facility.
CHILD ABUSE
South Fulton Studios takes child abuse very seriously and is required to report any suspected child abuse to the proper authorities.
TUITION AGREEMENT/ MAKE-UPS – no make-ups are provided if you miss days. In circumstances beyond the center’s control (inclement weather, local/national crisis, power outage or other emergency) the center will not provide make-ups, refunds or credits. Tuition is still due regardless of attendance or not. Tuition is still due in the event of vacation, illness, inclement weather, local/national crisis, power outage or other emergency.
HOLDING SPOTS - Full weeks tuition and registration fee is due to hold spot.
If student is NOT present at center due to illness or family emergency tuition is charged for 50% of the agreed upon, unless your child will not be able to return to the studio. At this time a two week notice is required upon discharge. If your child is present 1 day in a given week or more, full tuition will be charged.
SFS will not be open during the times specified on studio calendar. Please make additional arrangements as per studios calendar. SFS has the right to change it’s operation calendar. Tuition will be due during certain holidays and major performances and competitions.
Any future changes must be written and will only then amend this agreement as per all parties that have signed the original agreement.
**Once an invoice is two weeks past due, there will be an immediate termination of service and the total amount due will be passed on to the collection agency: USA CREDIT RECOVERY
***Debt collection’s 30% collection fee will be paid for by the debtor.
TRIP TRANSPORT AGREEMENT Parents must complete and sign all transportation agreements prior to scheduled field trips.
AUTHORIZED PICK UP
In the event the authorized person is not present to receive my child, Parent will indicate on application or email the following procedures are to be followed:
In the event that my child is not to be transported as outlined above, I agree to notify the South Fulton Studios immediately.
*Parent Must List on Dance Studio Pro Portal & Application Adults Authorized to Pick up Minor children.
EMERGENCY PROCEDURES
All precautions will be taken to prevent serious health risk to all students. In the event that a minor injury occurs,
First aid will be administered at the site by the staff. The following procedures will be followed:
First aid will be provided and the incident recorded in the Facility Health Log.
The child will be observed continuously and made as comfortable as possible during and following the administration of first aid.
In the event that a major injury or health problem arises and professional medical care is required, the following steps will be taken:
911 will be called & Immediate first aid will be administered by a staff person until professional services arrive. All efforts will be made by the staff to keep your child comfortable.
You will be contacted. If you cannot be reached, your emergency contact person will be notified.
A staff person will accompany your child to the hospital and remain until you or your emergency contact person arrives.
The incident will be described in writing on a South Fulton Studios Incident Form.
There are members of our staff that are trained in CPR and first aid. Please note: South Fulton Studios does not incur the cost of medical treatment and it is imperative that you indicate on your child's health history form what type of health insurance you carry.
In Case of Fire: In the event of a fire South Fulton Studios Students Students will walk to the Courtyard Stage going towards Campbellton Rd or the Annex Building behind studio. Parents will be notified by call or text.
In Case of Evacuation: Staff and students will report to the FairburnYouth Center Located : 149 SW. Broad St Fairburn GA 30213

In the event of an emergency involving my child, and if South Fulton Studios cannot get in touch with me, I hereby authorize any needed emergency medical care. I further agree to be fully responsible for all medical expenses incurred during the treatment of my child.
On South Fulton Studios Waivers & Policies I acknowledge by Clicking & Agreeing to all Policies & Waivers I acknowledging that have read agree and understand the South Fulton Studios Handbook. I acknowledge this satisfies my signature.
