I will read and adhere to all Villa West Dance & Acrobatics, LLC policies; and will take the responsibility to carefully follow the rules and policies therein. I understand that Villa West Dance & Acrobatics, LLC does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Villa West Dance & Acrobatics, LLC, and I voluntarily assume the risks associated with such participation.

MEMBERSHIP WAIVER
I have read and understand all Villa West Performing Arts Center policies; and will take the responsibility to carefully follow the rules and policies therein. I understand the Commitment & Tuition Policy and understand that Villa West Performing Arts Center does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc.

I understand that my child may be photographed or video recorded for training purpose, safety assurance, and/or possible advertisement. I may opt out of participation in advertisement (use of my Child's picture in advertising) by writing the front office.

I further understand that there are specific risks of physical or property damages, losses, or injury and illness that may result from my or my child’s participation with Villa West Dance & Acrobatics, LLC, and I voluntarily assume the risks associated with such participation.
