You must be over 18 to register for an account.

By enrolling my child in classes at Avenue D, I accept the following:

I understand that my child is only under supervision by Avenue D Performance Studio from the commencement to the completion of class and no responsibility is taken for my child outside of this period.

As a parent / caregiver, I understand that dance is a physical activity that requires correct technique, placement and alignment of the body and therefore, may require appropriate physical contact to enable correction throughout class tuition, and assisting with costumes as necessary.

I hereby absolve Avenue D Performance Studio from any liability in the event of any injury to my child. I give Avenue D permission to seek medical attention at the time of the accident or serious illness.

I understand that my child will be enrolled for the calendar year, with fees to be paid by the due date each term.

I acknowledge that I have read and agree to the 2025 Info Pack.

I understand that classes not attended will not be refunded unless the class has been cancelled. There will be no rebates for classes missed due to holidays, injuries, sickness or other sporting or family commitments.

I give permission for photographs and videos taken of my child to be used for promotional and advertising purposes, as well as shared within the Avenue D community.
