I, the undersigned, hereby confirm that I am the custodial parent or legal guardian of the minor children
identified below (hereinafter the “Participants”). I hereby consent to the Participants being present at the
facilities of Dance Force Studios, LLC (hereinafter “Dance Force”) and taking part in any and all activities offered by Dance Force.
I acknowledge that, even though all activities offered by Dance Force are supervised by employees of
Dance Force, the activities offered, including but not limited to dance activities, involve inherent risk of injury. I
hereby waive, both for myself and the Participant, any claim of any kind or nature that either I or the Participant may have acquire against Dance Force LLC or its employees or agents arising out of or in any way related to the Participants’ presence at the Dance Force facility or performance by the Participants of activities offered by Dance Force or any injuries incurred due to the Participants’ presence at the Dance Force facility or performance by the Participants of the activities offered by Dance Force or any injuries incurring due to the participants’ presence at the Dance Force facility or arising out of the performance of those activities so long as such claim is not a result of gross negligence or willful misconduct on the part of Dance Force LLC or its employees and or agents.

COVID-19 Waiver of Liability

understands that the novel coronavirus (COVID- 19) respiratory infection has been confirmed and spread throughout the United States, including cases in Illinois, Wisconsin, Indiana and Iowa. In accordance with the most recent guidance and protocols issued by the World Health Organization (WHO) and the Centers for Disease Control and Prevention (CDC) and the Illinois Department of Health for slowing transmission of COVID-19, Dance Force LLC is putting new procedures in place to protect our dancers, families and staff. Furthermore, I understand that Dance Force LLC has put in place preventive measures to reduce the spread of COVID -19 while in the facility or participating in Dance Force LLC events; however, Dance Force LLC cannot guarantee that you or your child(ren) will not become infected with COVID-19.

I also acknowledge that it is my responsibility to monitor the health and wellness of my family and child(dren) and acknowledge that it is my responsibility to inform Dance Force LLC should I, or anyone in my immediate household have/had a direct exposure to any person who has/had a suspected or confirmed case of COVID-19. In addition, I understand and agree that if it occurs I, or my child, will not visit the facility for at least 14 days to minimize exposure to others at Dance Force LLC.

I understand Dance Force LLC cannot guarantee that you or your child(ren) will not become infected with COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID -19 and voluntarily assume all risk that my child(dren) and I may be exposed to or infected by COVID-19 by attending Dance Force LLC activities and that such exposure or infection may result in personal injury, illness, financial loss, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID -19 at Dance Force LLC may result from the actions, omissions, or negligence of myself and others and agree to indemnify, defend, and hold harmless Dance Force LLC and their staff against any and all costs, expenses, damages, claims, lawsuits, judgement, losses and/or liabilities (including attorney fees) arising either directly or indirectly from or related to any and all claims made by or against any of the released parties or otherwise specifically related to COVID-19.

By signing below I agree that I have read and acknowledge this waiver and I have been properly informed about the preventive measures Dance Force LLC is taking and the risks involved with participating in Dance Force LLC activities during and after the COVID-19 global pandemic.

Parental Photo/Video Usage Waiver:
By signing this waiver, I agree to allow Dance Force LLC to use and reproduction of photographs and digital images (Photo and video) taken of the children listed above for the purpose of marketing, internet marketing, public relations, and promotions.
