To the extent permitted by law, the person registering for classes if eighteen or older in age, or the legal guardian of the person registering for classes who is under eighteen in age, on behalf of said person, I do hereby waive any and all claims, release from all liability, and agree not to sue on behalf of myself, my children, my parents, my heirs, assigns, personal representatives estates as follows: Jazziak's Dance School, Inc, it's agents, officers, employees, students staff, or volunteers.
I accept all the inherent risks of participating at Jazziak's Dance School, Inc. I understand and agree that Jazziak's Dance School, Inc, it's agents, officers and employees shall assume no responsibility of liability to me for accident, or loss of, damage to, personal property resulting from participation in the Jazziak's Dance School, Inc activity.

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that Jazziak's Dance School, Inc. has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that Jazziak's Dance School, Inc. can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, studio staff, and other studio clients and their families.
I voluntarily seek services provided by Jazziak's Dance School, Inc. and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while participating in Jazziak's services.
I attest that:
* I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.
* I have not traveled internationally within the last 14 days.
* I have not traveled to a highly impacted area within the United States of America in the last 14 days.
* I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
* I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities.
* I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold Jazziak's Dance School, Inc. harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the studio, or that may otherwise arise in any way in connection with any services received from Jazziak's Dance School, Inc. I understand that this release discharges Jazziak's Dance School, Inc. from any liability or claim that I, my heirs, or any personal representatives may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Jazziak's Dance School, Inc.. This liability waiver and release extends to the studio together with all owners, partners, and employees.

I hereby grant permission to the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration. I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to insect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording.
By signing this release I understand this permission signifies that photographic or video recordings of me may be electronically displayed via the Internet. I will be consulted about the use of photographs or video recording for any purpose other than those listed above.
There is no limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.
By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release any and all claims against any person or organization utilizing this material for educational or advertising purposes.

I have read, understand, and agree to the policies and procedures of Jazziak's Dance School, Inc. as listed in the handbook and website Jazziaks.com. Failure to abide by these policies and procedures may result in termination from this school.
