TUITION INSTALLMENTS. The tuition for our Annual Season is split in two installments. 50% is collected upon enrollment. The remaining 50% will be due on December 1.

NO REFUNDS. Tuition is non-refundable.

INJURY WAIVER AND MEDICAL RELEASE. I understand there is a risk of personal injury associated with ballet classes, rehearsals, and performances. I affirm that the student is in good health and physically capable of participating in ballet classes, rehearsals and performances. I release, discharge and indemnify Rye Ballet, LLC ("Rye Ballet"), and its owners and faculty from any claim that may arise out of a personal injury that occurs during or in connection with ballet classes, rehearsals or performances in or around the dance studio or at a location of rehearsals or performances. I accept responsibility for obtaining the appropriate insurance to cover the student in the event of personal injury. If I cannot be reached, I give my consent for Rye Ballet and its faculty to seek any medical assistance required and I agree to be responsible for all medical expenses incurred on behalf of the student.

PHOTO RELEASE (OPTIONAL): I consent to the use of photographs and videos of my child by Rye Ballet for advertising and promotion in all media including but not limited to print and internet. I understand and agree that all photos and videos of students during ballet classes, rehearsals and performances are the property of Rye Ballet. I agree to hold harmless, release and discharge Rye Ballet, its owners and faculty from all claims, demands and causes of action in connection with any misuse or unauthorized use of such photographs and videos.
