STUDIO POLICIES

Studio Overview: The goal of Christian Dance Alliance (CDA) is to provide our students with quality, age-appropriate dance instruction in a positive, nurturing Christian environment. Our curriculum integrates Christian principles and values with classical dance technique training. The heart of this ministry is sharing the joy of the Lord through the art of dance. To ensure the best possible experience for your child, please review the following information and contact Sarah Schatz, Director at info@christiandancealliance.com or (513) 374-0555 if you have any questions.

Registration: Pre-registration is required. Full payment for current semester is due at the time of registration. Please register based on your child’s age and technical level at the start of the session for which you are registering. Class sizes are limited. Enrollment is first-come, first-served. Four students minimum required per class. If a class is cancelled due to low enrollment, a full refund will be processed. Schedule and instructors subject to change.

Studio Location: Our classes are held at Five Seasons Sports Club, 11790 Snider Road, Cincinnati, OH 45249. Five Seasons has graciously allowed us to rent their facility and use their studios. Please be respectful to other patrons and all areas of the Five Seasons facility.

Waiting Areas: Our classes are held at Five Seasons Sports Club, 11790 Snider Road, Cincinnati, OH 45249. Five Seasons has graciously allowed us to rent their facility and use their common areas as our waiting areas. Please be respectful to other patrons and all areas of the Five Seasons facility. The lower and upper-level lobby areas and the upper-level café area may be used as waiting areas for parents/siblings provided they are not being used by Five Seasons staff or members. Only members of Five Seasons Sports Club may use the running track, exercise equipment, and water/coffee bar in the main lobby. Dancers and other children may not be in the studio rooms unless supervised by an instructor. Children MUST be supervised at all times. Please clean up after yourselves and your children, and report any issues or damage to the Director immediately.

Spectators: In order to provide a safe and productive environment for our students, CDA does not allow spectators in any of our classes except during observation day classes. Each studio has windows if you would like to observe from time to time. Please do not linger at the windows/doors as that could become a distraction for your child and our other students. Please also ensure that siblings are not knocking on the doors or windows.

Parking: There are two parking lots adjacent to Five Seasons Sports Club – the upper lot and the side lot. Please do not park under the portico by the main entrance. Please drive with caution and supervise your children in the driveway and parking lot area at all times. The parking lot is monitored by video surveillance. Please lock your vehicle and do not leave valuables visible. CDA and Five Seasons Sports Club cannot be held responsible for items lost or stolen.

Security: For everyone's safety, parents should walk their children into the building and come inside to pick them up. Children may be released to someone other than a parent only after the instructor has received written or verbal permission from that child’s parent and/or guardian. Please do not allow children to walk through the parking lot unattended. Students must be picked up immediately following class. We cannot provide childcare for our students outside of their registered class(es). Reoccurring late pick-ups will result in dismissal from our studio programs.

Belongings: Students should place their belongings (dance bags, cell phones, water bottles, etc) inside the studio along the back wall or in the cubbies. Please do not bring valuables, and do not leave your belongings unattended at any time. With the exception of medical devices, cell phones and other electronic devices are not permitted in class (including during water breaks), and must be turned off or to “silent” mode and stored inside students’ dance bags. Please label all your belongings, especially dance shoes. CDA and Five Seasons Sports Club cannot be held responsible for items lost or stolen.

Placement & Promotion: CDA is committed to the technical, artistic, and spiritual growth of each student. All students are placed according to age and skill level as determined by our instructors. Students will not necessarily advance to the next level on an annual basis. Several factors are considered when deciding what level to place each student in including technical ability, strength, age, maturity, attendance, and commitment. To ensure safe execution of movement, proper body alignment, strength, and flexibility must be achieved before undertaking more complex exercises. Please do not be concerned if your child repeats a level of training; this is normal.

Attendance: In order for a student to reach their fullest potential, they must commit to regular class attendance. Please be advised that attendance will be monitored. Missed classes will not only slow your student’s progress, but the progress of the entire class. In some cases, missed classes may result in limited performing opportunities. If you will be absent from class, please email attendance@christiandancealliance.com to notify us in advance. Please review our Wellness Policy for information and protocol regarding illnesses.

Make-up Classes: Make-up classes are only available when the studio is notified of the absence prior to the class start time and when there is another class with the same age and level of students available. We cannot guarantee there will be a make-up class option available for all of our classes. No refunds will be given for missed classes.

Arrival/Dismissal Etiquette: Please arrive at least 5-10 minutes prior to class to prepare for class and change your shoes. Please wait in the studio lobby area until your instructor invites you into the studio. Prior to class, do not enter the studio without your instructor's permission. If you arrive after your class begins, please enter the studio, but wait quietly by the door for the instructor to invite you to join the class. For the safety of the dancers, arrival more than 15 minutes after the start of class may result in the student being asked to observe class rather than participate. Please remember that tardiness is a distraction to the entire class. If a student must leave prior to the end time of class, the instructor must be notified prior to the start of class.

Dress Code: It is extremely important for our instructors to be able to adequately see students’ bodies in order to promote proper body alignment and safe execution of movement. We strongly support modesty in our studio; however proper dance attire, without excessive over-clothing is required. If a student shows up without the proper attire or shoes, they will be asked to sit and observe rather than participate in class. No refunds will be given for missed classes due to improper attire or shoes. No jewelry, with the exception of stud earrings and flat rings, is permitted. Please do not wear dance shoes outside on pavement. Pebbles, rocks and dirt will damage the professional dance floors. Hair must be pulled completely off the face for all classes. Please review the dress code handout for information specific to your class(es).

Inclement Weather: For the safety of our dancers and families, the following inclement weather policy will be followed. For winter weather conditions, if Sycamore Schools are delayed, all morning classes will be cancelled, but afternoon and evening classes will continue as scheduled. If Sycamore Schools are closed, all morning classes will be cancelled; however, the cancellation of afternoon and evening classes will be determined by 2pm. In all cases, you will be notified about the status of classes by email. In the case of a tornado warning in Hamilton County, all classes will be cancelled. Please use discretion if you are traveling in inclement weather. We will do our best to provide notification via email if classes are cancelled due to inclement weather. You are always welcome to text/call the Director at 513-374-0555 to confirm if classes are in session. We do not credit, refund or pro-rate tuition for classes cancelled due to inclement weather. We will make every attempt to offer a make-up class for classes cancelled due to inclement weather; however, this may not always be possible due to scheduling constraints.

Communication: CDA will use email or text via the DanceStudio-Pro Parent Portal, as the main sources of communication. Please make sure your contact information is correct in the system.  By registering for classes, I acknowledge that I have read, understood, and agree to all Studio Policies.

 
TUITION & FEE POLICIES

Payment: 100% of current semester tuition is due at the time of registration. Payment of tuition secures enrollment in your class(es) for the specified course session. Tuition payments can be made by check, or credit card. Checks should be made out to CDA or CHRISTIAN DANCE ALLIANCE. Check payments should be mailed to Christian Dance Alliance, 4063 Saint Andrews Court, Mason, OH 45040. Please do not mail cash. If your account is past due, you will be unable to participate in class(es) or performances until the account is brought current. All accounts must be paid in full by May 1st otherwise performing will be restricted.

Refunds: All refunds will be handled on a case-by-case basis at the sole discretion of the Director. CDA reserves the right to not refund any or all tuition. If a class is cancelled due to low enrollment, a complete refund will be provided. Absences from class do not affect tuition payments. No refunds will be given due to a student missing classes.

Withdraw: In the event a student needs to withdraw prior to the completion of the session, the Director must be notified in writing so CDA can close out your account. Refunds for early withdraw will be evaluated on a case-by-case basis at the sole discretion of the Director. 30 days-notice is required. No refunds will be given for classes already taken. All Production fees are non-refundable.

Scholarships: A limited number of financial-need scholarships are available. Please contact the Director for a confidential scholarship application. Scholarships will be evaluated and administered on a case-by-case basis at the sole discretion of the Director.

Production Fee: In addition to tuition, each student must pay a Production Fee to participate in our Spring Dance Production (recital). Spring Production Fees include the student’s costume, tights, t-shirt and other production related expenses (facility, programs, staffing, etc). Students enrolled in multiple classes will pay a reduced Production Fee for each additional class. Current Spring Dance Production Fees are $120 for the student’s first class, $75 for each additional class. Fees are subject to change.

Returned Check Fee: A $25.00 returned check fee will be charged on all insufficient fund checks.

 By registering for classes, I acknowledge that I have read, understood, and agree to all Tuition & Fee Policies.

MEDIA RELEASE

I hereby authorize CDA to photograph and/or video me or my minor child(ren) in dance classes, camps, workshops, performances, or other CDA sponsored events; to edit any and all media at their discretion; and to use any and all media for promotional materials including print, email, and web publications. I acknowledge and agree that all rights, title and interest in said media shall be and remain the sole and exclusive property of CDA. I do hereby release any and all rights, claims or interests I may have in, to or upon said media.

Further, I do hereby, release and forever discharge CDA and its members, employees, and directors from and against any and all action, causes of action, suits, costs, claims, demands, expenses, liability and obligations, for upon or by reason of any damage, loss or injury, to persons or property which may arise from, be sustained, suffered or incurred in consequence of CDA’s use of said media.

 By registering for classes, I acknowledge that I have read, understood, and agree to the Media Release.

 
LIABILITY RELEASE

PLEASE READ THIS CAREFULLY. It affects any rights you may have if you or your minor child(ren) are injured or otherwise suffer damages while voluntarily participating in Christian Dance Alliance (CDA).

I consent to my or my minor child(ren's) participation in CDA. I understand that CDA is a dance organization and that dance training and performance involve strenuous physical activity. I certify that I understand the activities associated with my or my minor child(ren's) involvement in CDA, and that I or my minor child(ren) are healthy and able to undertake the physical activity required to participate in CDA. I assume full responsibility for any risk of loss, loss of property, property damage, or personal injury that may be sustained by me or my minor child(ren), or any loss or damage to property owned by me or my minor child(ren) as a result of participation in CDA. I will ensure that I and my minor child(ren) follow all safety rules established for CDA, and I assume full responsibility for failure of myself or my minor child(ren) to do so.

I hereby, RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO PURSUE LEGAL ACTION AGAINST Christian Dance Alliance, and any class, performance, or rehearsal location at which I or my minor child(ren) undertake any activity associated with CDA, its members, employees, and directors and any of the officers, servants, agents and employees of the above-mentioned entities (hereinafter referred to as RELEASEES) for any liability, claim and/or cause of action arising out of or related to any loss, damage, or injury, including death, that occurs as a result of my or my minor child(ren's) participation in CDA.

I agree TO INDEMNIFY AND HOLD HARMLESS THE RELEASEES whether or not injury or damages is caused by my or my minor child(ren's) negligence, the negligence of the RELEASEES or the negligence of any third party. I further agree that this Release and Hold Harmless Agreement shall bind the members of my family and spouse, and my heirs, assigns, and personal representative (if any), and shall be deemed as a RELEASE, WAIVER, DISCHARGE AND COVENANT NOT TO PURSUE LEGAL ACTION AGAINST the above-named RELEASEES. I hereby further agree that this Release and Hold Harmless Agreement shall be construed in accordance with the laws of the State of Ohio.

 By registering for classes, I acknowledge that I have read, understood, and agree to the Liability Release.


MEDICAL RELEASE

I understand that I or my minor child(ren) will be participating in strenuous physical activity that may place unusual stress on my or my minor child(ren's) body and with that the risk of physical injury. I confirm that I or my minor child(ren) are in the appropriate physical condition to participate. I also confirm that I have listed any medical conditions and allergies on my and my minor child(ren's) student profile. I will notify CDA immediately if I or my minor child(ren) develop any physical conditions, health problems, injuries, or allergies so my profile can be updated.

I hereby give my permission to CDA personnel to authorize or administer any minor emergency medical treatment that I or my minor child(ren) may require during any and all CDA classes, camps, workshops, intensives, performances or events. I understand that I am responsible for any and all charges as a result of such care and medical treatment.

 By registering for classes, I acknowledge that I have read, understood, and agree to the Medical Release.


AUTHORIZATION AND CONSENT

I acknowledge that I have read and understood all of the release statements, policies and procedures, tuition and fees associated with dancing at Christian Dance Alliance (CDA).

KNOWING AND VOLUNTARY EXECUTION

I have carefully read and fully understand the contents and legal ramifications of this agreement. I understand that this is a legally binding and enforceable contract and sign it of my own free will. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect to the fullest extent permitted by law.

PARENTAL OR LEGAL GUARDIAN CONSENT REQUIRED FOR MINORS

If the person(s) identified on the undersigned’s Christian Dance Alliance account is under 18 years of age, this agreement is being executed by either a parent or legal guardian of the minor, who, through such execution, also releases and indemnifies Christian Dance Alliance and the Released Parties to the fullest extent provided above.

For Dance Combo 1/2, girls should wear a leotard and tights (any color/style) and PINK ballet slippers. Skirt is optional. Boys should wear a WHITE t-shirt, BLACK shorts, and BLACK ballet slippers. The first few pictures to the left show great examples of what our youngest girls should wear. We recommend our young dancers bring an extra change of clothes and a clearly labeled water bottle. If necessary, wear a pull-up. Hair should be neatly secured off of the face, preferably in a ponytail or bun.

For Pre-Ballet & Jazz Combo 1/2/3, girls should wear a leotard and tights (any color/style) and PINK ballet slippers. Skirt is optional. Male dancers should wear a WHITE t-shirt with BLACK leggings and BLACK ballet slippers, and BLACK pull-on jazz shoes. All visible undergarments should be BLACK. Tights must be worn over the feet and inside the shoes. Shoe elastics must be properly sewn. Ballet skirts or BLACK fitted dance shorts may be worn at instructor discretion. Proper dance warm-up clothing (such as ballet sweaters, leg warmers, leggings) may be worn until the end of tendu combinations, but clothing should not be restrictive to movement or too bulky as that will prohibit corrections to body alignment and technique. See pictures on left. Hair must be neatly secured off the face and neck in a bun.

For Ballet Level 2/3/4/5/6, female dancers should wear a BLACK leotard (any modest style), PINK tights (convertible style), and PINK split-sole, preferably leather ballet slippers (such as Capezio Hanami #2037 or Eurotard Coupe #A2004). Male dancers should wear a WHITE t-shirt with BLACK leggings, and BLACK ballet slippers. All visible undergarments should be BLACK. Tights must be worn over the feet and inside the shoes. Shoe elastics must be properly sewn. Ballet skirts or BLACK fitted dance shorts may be worn at instructor discretion (no athletic shorts). Proper dance warm-up clothing (such as ballet sweaters, leg warmers, leggings) may be worn until the end of tendu combinations, but clothing should not be restrictive to movement or too bulky as that will prohibit corrections to body alignment and technique. For Pointe, see dress code for ballet above, plus pointe shoes properly fitted with necessary toe pads, gauze, lamb’s wool, band-aids, and/or tape to protect toes. New pointe shoes must be approved by the Instructor or Studio Director prior to sewing ribbons and elastics.

For Contemporary/Modern, dancers should wear a leotard (any color/style), TAN or PINK tights (convertible style), and SKIN-TONE (please select the closest color to your skin tone), HALF-SOLE, preferably LEATHER lyrical “pirouette” shoes (such as Capezio Style #H062 leather or #H064 canvas). Modern dancers also need crew-length BLACK socks (can be purchased at H&M near the register). BLACK dance shorts, capris, or leggings may be worn at instructor discretion, but clothing should not be restrictive to movement or too bulky as that will prohibit corrections to body alignment and technique. Hair must be neatly secured off the face and neck in a bun. Kneepads are recommended.

For Jazz 2+, dancers should wear a leotard (any color/style), TAN or PINK tights (convertible style), and SKIN-TONE, HALF-SOLE, preferably LEATHER lyrical “pirouette” shoes (such as Capezio Style #H062 leather or #H064 canvas). BLACK dance shorts/capris/leggings may be worn, but clothing should not be restrictive to movement or too bulky as that will prohibit corrections to body alignment and technique. Hair must be neatly secured off the face and neck in a bun.

For Tap, dancers should wear modest work-out attire that allows for freedom of movement. Clothing should not be restrictive to movement or too bulky as that will prohibit corrections to body alignment and technique. Midriff should not be exposed. If necessary, dancers should wear a leotard (any color/style). Dance shorts/capris/leggings may be worn, but no shorts without tights. Required shoes: MATTE BLACK, TIE-UP, LOW HEEL tap shoes (such as Capezio Downtown Tap Shoe #473). Hair must be neatly secured off the face and neck.

For Hip Hop, dancers should wear modest work-out attire that allows for freedom of movement. Clothing should not be restrictive to movement or too bulky as that will prohibit corrections to body alignment and technique. Midriff should not be exposed. If necessary, dancers should wear a leotard (any color/style). Dance shorts/capris/leggings may be worn, but no shorts without tights. Required shoes: TBD, please wear clean tennis shoes/sneakers until further advised. Hair must be neatly secured off the face and neck.

For Worship Dance, dancers should wear modest work-out attire that allows for freedom of movement. Midriff should not be exposed. If necessary, dancers should wear a leotard (any color/style). Dance shorts/capris/leggings may be worn, but no shorts without tights. Ballet slippers, dance paws, socks or bare feet are acceptable. Hair must be neatly secured off the face, unless otherwise directed by the Instructor.

*Note on Ballet Slippers: please ensure that you purchase actual ballet slippers (see image). Some retail stores sell house slippers marketed as ballet slippers. These are not acceptable for class and are slippery. We recommend leather ballet slippers as they last longer. Shoes and other dress code items can be purchased at Diana’s Dancewear in Blue Ash or VeraNova in Mason. Please do not purchase ballet slippers at Target as the color and quality are not good.

*Note on Dance Attire: all dance attire should be modest and professional in appearance. Instructors reserve the right to require modifications to a dancer’s apparel. Repetitive failure to follow the dress code may result in a student being asked to observe rather than participate in class.

DANCE CLASS ETIQUETTE

All dancers are expected to respect their teachers, their fellow dancers, and their art form. Appropriate dance class etiquette is expected at all times. Please review this document so you understand the appropriate etiquette. Self-discipline in these areas demonstrates your maturity and desire to learn and progress.
Foundational Guidelines for All Classes

1. Listen and Obey (Right Away, All the Way, with a Happy Heart/Good Attitude)
- Ephesians 6:1 “Children, obey your parents [teachers] in the Lord, for this is right. ‘Honor your father and mother’ —which is the first commandment with a promise— ’so that it may go well with you and that you may enjoy long life on the earth.’”
- Philippians 2:14-15 “Do everything without grumbling or arguing, so that you may become blameless and pure, children of God without fault in a warped and crooked generation. Then you will shine among them like stars in the sky.”

2. Do Your Best
- Colossians 3:23-24 “Whatever you do, work at it with all your heart, as working for the Lord, not for human masters, since you know that you will receive an inheritance from the Lord as a reward. It is the Lord Christ you are serving.”
- 2 Timothy 2:15 “Do your best to present yourself to God as one approved, a worker who does not need to be ashamed and who correctly handles the word of truth.”

3. Encourage One Another
- 1 Thessalonians 5:11 “Encourage one another build each other up, just as in fact you are doing.”
- Ephesians 4:32 “Be kind and compassionate, forgiving each other.”

4. Have Fun

5. Dance for Jesus

- Psalm 30:11 "You turned my wailing into dancing; you removed my sackcloth and clothed me with joy."


Lobby, Hallway, and Restroom Etiquette
• Appropriate language and topics of conversation are expected at all times.
• Harassment or bullying of any kind will not be tolerated, and should be reported immediately.
• Please exercise respect for the personal property of others at all times. Do not use or touch other people’s property without permission, this includes but is not limited to electronic devices.
• Please respect the rules of the Five Seasons facility. All exercise equipment and the track are for member use only.
• For your safety, stay off the running track at all times, unless walking into the studio.
• Respect patrons that may be using the running track, coming in/out of restrooms or studios or up/down the stairs.
• Do not block doorways, stairways or hallways, with personal bags, shoes or other belongings.
• Do not change your clothes in the hallway, and do not monopolize the restrooms for excessive periods of time.
• In the restrooms, refrain from using any unnecessary items such as extra towels, hairspray, soap, or deodorant.
• Any trash must be discarded in the trash cans provided. In the event of a spill, please clean it up and notify an Instructor.
• Proper outer wear (pants, shirt, sweater, or coat) should be worn when leaving the studio. Older students should not wear only a leotard and tights out in public; please wear overclothing.

Pre-Class Etiquette
• Please arrive 10 minutes before class to warm up and prepare for class.
• Before entering the studio you should be properly dressed for class. Please refer to the dress code handout.
• All dance attire, hair and personal needs, and restroom breaks must be taken care of before class begins.
• When invited by the Instructor, enter the studio quietly and take your position at the barre or on the floor.
• Any injuries or physical limitations should be reported to the Instructor prior to the start of class.
• Before class begins, take a moment to reflect on why you are in class and prepare your mind, and body, and spirit for class (Matthew 22:37-39, 1 Corinthians 6:19-20).
• If arriving late, quietly step inside the classroom and remain by the door until the Instructor invites you to join the class.
• If you are more than 10 minutes late, for your safety and out of respect to your fellow dancers, you may be asked to sit out and observe class. Repeated tardiness will be addressed by the Instructor/Director.
• If you must leave early, the Instructor should be notified prior to the start of class. Please depart without causing distraction, but do receive the Instructor’s acknowledgement before leaving.

Classroom Etiquette
• Any items brought into the studio should be kept along the back wall of the room and should not block any of the doors.
• Jackets, coats, boots or other larger items should be placed out of the way close to dance bags.
• All electronic devices and cell phones must be turned off or on silent during class time, and may not be out during class.
• Once class begins, please show respect to your Instructor by politely responding with “please” and “thank you.”
• Listen and keep eyes focused on the Instructor as she/he demonstrates the combinations.
• Please refrain from talking while combinations are being given.
• Save all questions until the end of the combination demonstration, as your question may be answered before you ask.
• Remain alert and attentive throughout barre and center work.
• Listen and apply corrections with a polite response. Understand that corrections are given to help you improve.
• Unless injured or ill, students are expected to participate in all combinations during class, giving their best effort at all times.
• Do each combination all the way through with care and proper execution. Stopping before the end of a combination is counterproductive, potentially dangerous to other dancers, and rude. Unless injured, a dancer would never cut short a variation on stage at any time, the same attitude and effort is expected in class.
• Do not strain your muscles during your dancing. Use only the needed muscles and relax the others.
• Remember, every exercise is intended to strengthen and improve your dancing, and that the goal is not how high, fast or long, but how your dancing can progress into a harmonious language of movement.
• Facial expressions should be relaxed and pleasant as you are dancing.
• Be wise about your dance space, planning ahead based on the combination.
• Be aware of other dancers around you, and respect their dance space.
• You may need to adjust where you stand in order to make room for others.
• Please refrain from chewing gum, eating, or drinking in class (except water). NO FOOD IS ALLOWED IN THE STUDIO ROOMS.
• If you need water, be sure to bring a non-glass, closeable, and labeled water bottle into the classroom.
• No student should leave class without first informing and thanking the Instructor. This includes bathroom breaks.
• Please celebrate your work in class and honor your Instructor by clapping and thanking her at the conclusion of class.

Dance Bag Etiquette
• Remember to place clean dancewear into your bag each day/week.
• Your dance bag should contain:
- Clean leotard (appropriate color for your class)
- Clean tights (free from holes and runs)
- Clean dance shoes (all styles needed for your classes: ballet slippers, pointe shoes, jazz shoes, contemporary shoes, dance paws, modern socks, tap shoes, hip hop shoes, etc.) and relevant accessories (toe pads, stitch kit, etc.)
- Dance warm-ups and ballet skirts (if necessary)
- All necessary items to make your hair neat and tidy (bobby pins, hairnet, brush/comb, and hairspray)
- Band-aids and other personal care items
- Water bottle (non-glass, closeable, and labeled with your name)

WELLNESS POLICY
EFFECTIVE 2024 09 03

Keeping dancers home when they are ill is essential to protect the health and wellness of CDA students, families and instructors. Out of respect and care for one another, we ask that you commit to following the guidelines set forth in this Wellness Policy and exercise an abundance of caution to help keep our dance community healthy. Our instructors and staff will closely monitor the health of our students while at dance. Students should be well enough to participate in all classroom activities. Any student exhibiting symptoms of illness will be isolated, and the parent will be contacted to pick up the student immediately. Instructors and staff will abide by this same policy.

Prior to arriving for class:
• Review any new documentation provided by CDA, policies and procedures are subject to change.
• If the answer to any of the following general health screening questions is “yes,” do not send your dancer to class:
- Has the dancer had a temperature of ≥100° F in the past 24 hours?
- Has the dancer had vomiting or diarrhea in the past 24 hours?
- Does the dancer have any Covid-19 related symptoms?
o Cough, difficulty breathing, shortness of breath, fatigue, chills, shaking, body aches, muscle pain, new loss of taste or smell, congestion, headache, sore throat, runny nose, nausea, vomiting or diarrhea.
- Has the dancer tested positive (or is awaiting test results) for Covid-19? Please follow current CDC guidelines and monitor symptoms.

• If the dancer has any of these other maladies, do not send your dancer to class:
- Dancer is “out of sorts” or inconsolable
- Mouth sores
- Pink eye (conjunctivitis)
- Ring worm
- Lice or scabies
- Nausea
- Rash or skin lesions that are undiagnosed or diagnosed as contagious
- Other potentially contagious illness such as, but not limited to:
• Flu-like symptoms
• Croup
• Strep
• Herpes virus
• Impetigo
• Roseola
• Thrush
• Chicken Pox, Measles, Mumps, Rubella, Pertussis
• Hand, Foot, Mouth disease (coxsakie virus)
• Other undiagnosed illness

If the dancer has been sick, prior to returning to class:
• Answers to above general health and Covid-19 screening questions must be “no.”
• Dancer must not be exhibiting symptoms of illness or maladies listed above.
• Dancer should not return to classes until she has been free of illness and fever for at least 24 hours without the use of fever-reducing medications.
