Liability
I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all rights and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Inspired to Move Dance Academy, it's officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as "Inspired To Move Dance Academy").
I hereby agree to release Inspired to Move Dance Academy and hold Inspired to Move Dance Academy harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.
I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
I have received or will obtain a copy of Inspired to Move Dance Academy policies and will take the responsibility to carefully read and follow the rules and policies therein. I understand that Inspired to Move Dance Academy does not give credit and/or refunds for class(es) missed due to holiday, vacation, illness, weather, etc. I further understand that there are specific risks of physical or property damages, losses, or injury that may result from my or my child’s participation with Inspired to Move Dance Academy, and I voluntarily assume the risks associated with such participation.
If I am a minor, my parent and/or legal guardian has also signed this document releasing Inspired to Move Dance Academy from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.
The participant has my permission to participate in Inspired to Move Dance Academy. I release Inspired to Move Dance Academy of all liabilities associated with my child's attendance at Inspired to Move Dance Academy.
Tuition:
I understand that tuition is due by the first of every month. I understand that there will be a late fee charged if I do not pay by the 10th of the month, every month. I have had time to fully read and understand all tuition guidelines below and agree to comply with everything stated.
Tuition Guidelines:
Tuition is due by the 1st of the month. A late fee of $25 will be assessed on the 11th of the month for all delinquent accounts. There will be a fee of $35 for all returned checks.
A yearly registration fee of $15/child or $25/family is assessed in September or with your first month of tuition when you register September through June.
If you sign up after the first week of classes for the month, your tuition will be prorated for the remainder of the month. However, if you drop a class in the middle of the month there will be no refunds or credits given.
Tuition is based off of a twelve month calendar year. You have the option to pay for the entire year all at once or to have it divided into twelve equal monthly payments.
There will be dates and times that the studio is closed. We do not prorate for dates & times the studio is closed.
We do not refund for missed classes. You are always welcome to schedule a make-up class if you have missed. Please see the front desk to discuss your options.
We do not refund for cancelled classes. We do everything in our power to keep this from happening, but sometimes this is out of our control. If we cannot find a sub and the class must be cancelled, you are always welcome to schedule a make-up class. Please see the front desk to discuss your options.
Media:
I understand that Inspired to Move Dance Academy offers recreational activities. Many families choose to take photos during these classes. At times, I also understand that Inspired to Move Dance Academy will take pictures/videos for promotional purposes. I hereby give Inspired to Move Dance Academy permission to use my child's picture and/or video in or on any form of advertisement for Inspired to Move Dance Academy or an Inspired to Move Dance Academy affiliated event, including their website and social media pages. I understand that pictures may be taken during classes or events and I agree to release my child's photos to Inspired to Move Dance Academy for use.
COVID:
I understand that Inspired to Move Dance Academy is doing everything in their power to stop the spread of COVID-19. I agree to release Inspired to Move Dance Academy, its owners, representatives, and all employees from any and all liability for the unintentional exposure or harm due to COVID-19. I understand that Inspired to Move Dance Academy, its owners, representatives, and all employees cannot be held liable for any exposure to COVID-19 caused by misinformation or the health history provided by each student.
I agree that I will not bring my child to class if they have any symptoms of COVID-19 or if they have been exposed to COVID-19. If anyone in my household travels outside the US or to any city that is considered to be a "hot spot", I will quarantine my child for 14 days upon our return. I agree to inform Inspired to Move Dance Academy if anyone in my household is exposed to COVID-19.
