Although all activities are performed with the students' safety as the utmost concern, all students are required to be covered under their own family's insurance policies. If an injury should occur, you will only be compensated through your own family insurance policy.
I understand that the nature of dance instruction involves kinetic corrections that may include physically touching a student as part of regular class work and rehearsals. Students or parents with any concerns in this area should speak with the Director BEFORE attending any classes.
I understand that there is a risk of injury to my child with the participation in any dance class or rehearsal. I certify my child is in good health and able to participate fully in physical activity such as dance. I understand that I am waiving my right to bring legal action and to assert any claims against Calera Dance Academy, LLC, its respective agents or employees, for any and all negligence, loss, liability, damage, or personal injury, including death, that may be sustained by the participant, while in or upon the premises or any premises under the control of Calera Dance Academy, LLC, its owners or agents, or en route to or from any said premises.
I hereby affirm and acknowledge that I am giving full permission to participate in a virtual learning dance program offered by Calera Dance Academy, LLC if that option is deemed necessary due to unforeseen circumstances by either the staff, weather events (i.e. tornado, flooding, blizzard, etc.), or national/international crisis. By using any resources provided, I acknowledge that I am taking on all risk associated with the participation in an activity taking place outside of the brick and mortar establishment of Calera Dance Academy, LLC. I understand that it is my responsibility to provide a safe, conducive, and (reasonably) open environment for each class that the above-named person(s) will participate in virtually. In consideration of myself, or the above named person’s participation in the virtual dance program, I, for myself, my heirs and assigns, hereby release Calera Dance Academy, LLC, from any and all future rights, claims, causes of action, civil or criminal claims, demands, costs, attorney fees, loss of service, expenses, compensation, third party actions, suits of law or equity (including suits for contribution and indemnity), of whatever nature, and all consequential damages on account of, or in any way associated with the Calera Dance Academy, LLC.
I also understand that by agreeing to these terms, there will be no refunds, pro-rations, or credits given for any reason; nor will an account be suspended until such time that virtual learning is deemed unnecessary.
By agreeing to these terms, I hereby affirm that I have read and fully understand the foregoing release and know the contacts thereof, and hereby binds all parents and/or guardians of any minor participant.
