I certify that myself and/or my child/children is/are physically able to participate in dance, cheer, Tumbling/acro, &/or fitness classes. I certify that as with any activity, there is a certain amount of risk, & I forever waive & release Rayburn Dance Enterprises, LLC, DBA: Atascocita Dance Academy (ADA), Melissa & Michael Rayburn, & the entire ADA staff from any & all liability, damages, injuries, & loss of personal property.
I give my permission for my photo/image and/or my child’s/children’s photo(s)/image(s) and dance, cheer, tumbling/acro videos to be used on the ADA/CDA website & promotional items (flyers, brochures, ADA/CDA instagram, etc.).
I understand that registration fees are NON-refundable. I understand that tuition is NON-refundable once it is paid for the month (or semester/year if applicable). I understand that if I miss a class or it is canceled that I can take a make-up, but no tuition will be refunded.
Tuition is due on the 1st of each month. If tuition is not paid by the 5th, a $20 late fee will be assessed. Monthly tuition is considered 4 weeks of classes. The dates to which the tuition applies may not match up exactly with the calendar due to school holidays, 5-week months, etc. This is the same for split payments, which MUST be made by the 5th and 20th. There is also a $30 returned check fee.
I understand that costume deposits are NOT refundable as they are paid to the costume companies, who do not refund ADA/CDA. I understand that costumes are not made to order, so minor alterations will be at the expense of each parent. I understand that recital fees are NOT refundable.
I understand that profit from studio-sponsored fundraising can ONLY be applied to costume and recital fees. ADA Team members may also apply fundraising profit to regular team fees, not extra dances or any other fees. These will NOT be applied to tuition, banquets, guest teachers, etc.. I also understand that if I leave ADA/CDA, that fundraising profit STAYS at ADA/CDA, as it always belongs to ADA/CDA, and it is only earmarked for my use.
I understand that if ADA/CDA is closed for unforeseen circumstances, like weather, power outage, etc., we will try our best to make up classes, but we do not guarantee that each class will be made up. Make-up classes will be on different days/times than normal. I understand that if I miss a class or it is canceled that I can take a make-up, but no tuition will be refunded. The class you attend as a make-up may be different than the class you're enrolled in, which is fine as long as it's within your age/grade/level range. You have 3 weeks from the week you missed your class to make-up that class. And, you must check with us BEFORE you show up to a make-up class so we can let our teachers know & confirm there is space in the class for you to attend. There is a possibility that a make up class will not be possible, so it's best not to miss.
Once recital dances have been learned, you cannot make-up a recital class. You can come to a non-recital class as a make-up, like leaps & turns, acro, etc., provided there's room. Please note that if you have excessive absences in a recital class the 8-10 weeks prior to recital, you may need to attend a private lesson in order to participate in recital.
PLEASE come and speak with us if you need to make payment arrangements, we will do our best to accommodate you for extenuating circumstances that do not occur every month. Students accounts must be current/paid in full to participate in ADA/CDA Events/performances.
ADA TEAM MEMBER PARENTS
I understand that if I am delinquent with team payments, my child may not be able to participate in group dances. Extra dance fees will not be collected until all team payments are made.
