Mosaic Dance Center Policies and Release Agreements
TUITION PAYMENT POLICY: Payments are made in full or can be paid monthly, due by the 5th of each month September-June 1. Summer tuition is one set fee. All are payable the first day of class or before.Payment choices are cash, check, or credit card paid monthly, quarterly (Sept-Dec, Jan.-June) or in full.
Tuition is 36 weeks regardless of how many classes are in a month, for example, a 1 hour class is $612 for the season. Please Note: $10 late fee will be charged for each week following a 2 week grace period that the balance is not paid & students will not be allowed to participate in class. After 60 days past due, your account will be sent out for collections. If you need to drop classes at anytime, Mosaic must be notified 30 days in advance if a student drops or sign a drop form at the front desk. Tuition for the 30 days is still due until the drop period has ended or a $68 drop fee is due.
SUMMER TUITION: Is paid in full per session of classes on the first day of class. Make-ups are not allowed unless there is another class with space.
ADULT TUITION: Is paid in full per session of classes on the first day of class. Make-ups are not allowed unless there is another adult class with space.
MAKE-UP CLASSES. Make-up classes might be available for absences due to illness or other personal reasons during regularly scheduled classes. See front desk for list of make-up classes or availability. Mosaic Dance Center does not guarantee class availability for make-up.
WEATHER CLOSING: Mosaic does not follow any school system with regard to holidays, closings, teacher workdays, etc. For inclement weather or other closure information, please visit the web site at www.mosaicdancehall.com or visit Facebook.
DRESS CODE: . Please remember that all children’s classes have a dress code and shoe requirements, which are located on the web site at www. mosaicdancehall.com. All students must come to class in proper attire.
REGISTRATION FEE. The annual $30 Registration fee per student or $50 per family is non-transferable and non-refundable. Registration forms must be completed and signed for each student.
LATE FEE. $10 late fee will be charged for each week following a 2 week grace period from the 5tht of the month that a balance is not paid and students will not be allowed to participate in class or performances.
CLASS WITHDRAWAL AND REFUND POLICY. Early withdrawal from a class before the completion of the class requires a written notification and withdrawal form to Mosaic 30 days prior to withdrawal. No refunds or credits will be issued for missed classes or cancellation. Tuition will be charged for the last 30 days of class.
Class withdrawal requires a minimum of one FULL month advance notice. Notice MUST be given by completed withdrawal form available in the studio. Email, phone, and verbal communications are not official withdrawals. Monthly billing will continue until an official withdrawal form is received as per the policies agreed on upon submitting registration, regardless of whether the student is in attendance or not.
STUDIO POLICIES: Proper dance attire required for all classes. See class dress code description.
Hair must be securely tied back for all classes except adults.
A hair bun is required for ballet class.
No gum chewing or loose jewelry allowed during dance class.
No food or drink except water is allowed in the studio.
No street shoes are allowed on the studio floors.
Any dancer who is late and misses warm up in class, will not be allowed to participate in class until they have stretched on their own.
Disrespect of another student or a staff member will not be tolerated.
Costume fees are due in October 1 & January 15.
Tuition must be paid the 5th of each month and in full to continue in class. Late fees will be applied 14 days after.
Tuition or competition fees must be paid in full to participate in recital or performance.
Most dancers remain in each level for 2 years.
Dancers are moved up based on curriculum requirements.
Class size is based on studio space and the age of the dancers.
No soliciting of your business.
No conflict of interest will be tolerated. It is our intention to provide a positive, safe, and harmonious experience for all of our dance students. Because of this, we strongly encourage a spirit of community, cooperation, and caring, and we therefore discourage gossip and other negative or disruptive behavior.
Parent Code of Conduct: Parents have a responsibility to the staff at Mosaic Dance Center to be supportive of their decisions including rules policies. Do not undermine their efforts. If you do not agree with a coach then you are free to tell the coach but make sure it is done at the right time and place and not in front of the children or others. Parents are expected to drop off and pick up their children on time. At no time will a parent harass or falsely accuse a staff member. Mosaic Dance Center has the right to dismiss a child based on parents poor behavior or demands.
VIDEO/PHOTOGRAPHY RELEASE. The undersigned grants Mosaic permission to record the participation of the student on video or by photograph and to use it for promotion and public relations on the website (www.mosaicdancehall.com), in print, and at other Mosaic functions for promotional purposes only. Such recordings are the sole property of Mosaic Hall and Dance Center, LLC.
WAIVER AND RELEASE OF ALL CLAIMS.
In consideration of the services of Mosaic Hall and Dance Center, LLC their agents, owners, officers, volunteers, participants, employees, and all other persons or entities acting in any capacity on their behalf (hereinafter collectively referred to as MDC), I hereby agree to release, indemnify, and discharge MDC, on behalf of myself, my spouse, my children, my parents, my heirs, assigns, personal representative and estate as follows.
1. I acknowledge that my participation in aerial arts training and instruction, including acrobatic arts, aerial hoop and other disciplines entails known and unanticipated risks that could result in physical or emotional injury, paralysis, death, or damage to myself, to property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity.
The risk include, among other things: exposing its participants to the potential for slips and falls and falling; rope burns; pinches; scrapes, twists and jolts that could result in scratches, bruises, sprains, lacerations, fractures, concussions, or even sever life threatening hazards; strains, sprains, cuts, bruises, muscle soreness and fractures; musculoskeletal injuries including head, neck, and back; injuries to internal organs; the negligence of other people; my own physical condition; and the risk of emotional and psychological injuries or physical damage associated with this activity. Traveling to and from shows, meets, and exhibitions raises the possibility of any manner of transportation accidents. In any event, if you or your child is injured, you or your child may require medical assistance, at your own expense.
Furthermore, MDC employees and volunteers have difficult jobs to perform. They seek safety, but they are not infallible. They might be unaware of participant’s fitness, medical conditions, or abilities. They might misjudge the weather or other environmental conditions. They may give incomplete warnings or instructions, and the equipment being used might malfunction.
2. I expressly agree and promise to accept and assume all of the risk existing in this activity. My participation in this activity is purely voluntary, and I elect to participate in spite of the risks. I also acknowledge and accept the risks and dangers that come with choosing to participate in studio activities during the COVID-19 pandemic, including exposure, illness, injury, or death. I understand that adhering to the studio's new protocols and guidelines will significantly reduce these risks.
3. I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless MDC from any and all claims, demands, or causes of action, which are in any way connected with my participation in this activity, including my voluntary participation during the COVID-19 pandemic, or my use of MDC’s equipment or facilities, including any such claims which allege negligent acts or omissions of MDC’s.
4. Should MDC or anyone acting on their behalf, be required to incur attorney’s fees and costs to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs.
5. I certify that I have adequate insurance to cover any injury or damage I may cause or suffer while participating, or else I agree to bear the costs of such injury or damage myself. I further certify that I am willing to assume the risk of any medical or physical conditions I may have.
6. In the event that I file a lawsuit against MDC, I agree to do so solely in the state of Michigan, and I further agree that the substantive law of Michigan shall apply in that action without regard to the conflict of law rules of that state. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining document shall remain in full force and effect.
7. As additional consideration for the student’s instruction, the undersigned student, parent(s), or guardian(s) of the student hereby releases and waives any and all claims against Mosaic Hall and Dance Center, LLC and any and all of its employees, contractors, and volunteers for any liability including but not limited to personal and bodily injuries (including death) and loss of or damage to property of the student or persons related to the student which may occur while participating in the activities, activities sponsored by, or in the physical building of Mosaic Hall and Dance Center, LLC. The undersigned represents that the student is in good health and does not have any history of a medical or physical condition (unless specified above) that would place the student at risk because of his/her condition. The undersigned further acknowledges that the student’s instruction involves physical exercise and physical stress that could result in physical injury of the student, that the student’s participation is voluntary, and that the undersigned accepts all risks arising therefrom.
By choosing "Yes", I acknowledge that if anyone is hurt or property is damaged during my participation in the activity, I may be found by a court of law to have waived my right to maintain a lawsuit against Mosaic Hall and Dance Center, LLC on the basis of any claim from which I have released them herein. I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
By registering online I agree that II have read the policies and release agreements in their entirety, understand them, and agree to comply with their contents.
