I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that ESBD Music N Motion has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that ESBD Music N Motion can not guarantee that my child will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, studio staff, and other studio students and their families.
I voluntarily seek services provided by ESBD Music N Motion and acknowledge that I am increasing my risk and my child(ren) to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending classes.
I hereby release and agree to hold ESBD Music N Motion harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the studio, or that may otherwise arise in any way in connection with any services received from ESBD Music N Motion. I understand that this release discharges ESBD Music N Motion from any liability or claim that I, my heirs, or any personal representatives may have against the studio with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from ESBD Music N Motion. This liability waiver and release extends to the studio together with all owners, partners, and employees.

As the Parent or Legal Guardian of the child, I understand that as a result of his/her athletic participation, an injury could occur that may require medical attention. I further acknowledge that on the occasion the coaches and staff of ESBD Music N Motion and available medical personnel may be unable to contact me immediately for my consent for emergency medical care. I do hereby authorize the staff of ESBD Music N Motion to consent on my behalf as they may deem necessary to such emergency medical care, including ambulance transport and hospital care, as may be deemed necessary under the then existing circumstances by available medical personnel. In so doing I assume responsibility for the expenses of any such care not covered by my insurance.


As the parent or legal guardian, I give allowance of my child to attend and participate in the dance/gym classes conducted by ESBD Music N Motion. In doing so, I hereby stipulate and agree that for himself/herself, his/her executors, administrators, successors and assigns, release Kasandra Godman, ESBD Music N Motion, from any and all liability, claims, demands and causes of action arising from any personal injuries and property damages sustained by myself or his/her child or ward in, on or about the premises of ESBD Music N Motion. I understand the sum and substance of this release and assume full responsibility for injuries and or damages which may occur in, on or about the premises of ESBD Music N Motion. I expressly, fully and forever release and discharge Kasandra Godman, ESBD Music N Motion, her agents, employees and other students from any and all claims, demands, damages, rights of action or causes of action present or future, whether same be known, anticipated or unanticipated, resulting from or arising out of the children or ward’s use or intended use of the dance studio, facility and equipment located therein and the instructors and personnel located there at.
