In checking the box below I agree that CHARGES: I am responsible for all payments and fees for registration and monthly tuition. COPYRIGHT: All Dance Time music, materials, and curriculum are for the use of Dance Time LLC students and Dance Time LLC teachers only. Any replication or unintended use of items produced by Dance Time LLC is prohibited. MISSED CLASSES: There will be occasional make-up days available for students who have been absent. No refunds or discounted monthly rates will be available. RELEASE: By signing this contract you agree to release and forever discharge, Dance Time and their employees from any liabilities growing out of or related to any activities in which you may participate in. You understand that this is a full and complete release for all injuries or damages, which you may sustain as a result of your participation in any activities, regardless of the specific cause thereof. PHOTO RELEASE: Dance Studio will be taking photographs/videotape of your child during the course for publicity, promotional and/or educational purposes (including publications, presentations or broadcast via newspaper, internet or other media sources). I hereby grant Dance Time and its respective affiliates and licensees the right to use my child's photo in order to advertise, promote or publicize Dance Time and it’s affiliates. TEACHERS: Dance Time LLC is not responsible for class recruitment nor does it guarantee any monetary income. All rules and curriculum should be followed. In order to be considered "in good standing," teachers must be paying the monthly licensing fee and attend all required trainings. Dance Time LLC is not responsible for any liability for you, your studio, or your students. Any distribution of materials or operation of the Dance Time program when not in good standing with Dance Time LLC, can result in legal action. LIABILITY: Dance Time LLC or and Dance Time LLC teachers are not responsible for personal injury inside or outside of class.
