I, the undersigned parent or legal guardian of the dancer(s): __________________________________________________________, do hereby give permission for the aforementioned persons to participate in any and all classes, programs, shows, and events offered by or attended by Indiana Dance Company. I hereby accept all risks associated with that participation and understand that there is a full possibility of physical illness or injury. Participant, on his or her behalf, and the parent/guardian, on behalf of Minor/Participant as well as on their own behalf, unequivocally agree to incur and assume such risks as a condition to participation in the activities for which the Participant is registered.
In the event of illness or injury, I authorize Indiana Dance Company to obtain necessary medical treatment for the Participant and hereby, in my own behalf and on behalf of the Participant release and hold harmless all Releasees.The undersigned represents and agrees that this waiver and release is binding not only on the undersigned, but also upon their respective heirs, representatives, administrators, executors, and assigns. This release covers every possible injury or accident of every sort and nature, whether related to the permitted activity, the equipment, the condition of the premises, or otherwise, and regardless of whether due in whole or in part to the negligence of a Releasee or other Participant.

I give permission for my child’s or my picture to be used in publicity and school publications. I understand that this may include Internet based advertising and the dance school website. I will provide a letter or email to Indiana Dance Company (indianadancecompany@gmail.com) if I do NOT wish to give my consent for use of photography.

Liability
Indiana Dance Company Liability Waiver
and Acknowledgment of Risk:

I understand and agree that in participating in any dance class, workshop, rehearsal or performance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my child during any of Indiana Dance Company classes, rehearsals, performances, or activities. I also exempt, release, and indemnify Indiana Dance Company, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or property which may arise out of or in connection with participation in any classes or activities conducted by Indiana Dance Company. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold Indiana Dance Company, its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. If I am signing this waiver for my children, I certify that I am the parent or legal guardian and have the right to waive these rights.
Permission is granted Indiana Dance Company to use photographs and video of students for publicity purposes.

Policies
Safety
All young children must be under the supervision of a responsible parent or guardian; IDC staff is NOT responsible for your child unless during scheduled class time. Students must be picked up from the studio within 15 minutes of the conclusion of their classes, unless other arrangements have been made in advance.

Dance Attire
Please wear fitness apparel to class. Shorts, leggings, sweats, t-shirts, etc. NO DENIM!

Tuition
Your payment should be made to Indiana Dance Company. Payment is due the first of every month. If payment is not received by the 10th of each month a $10.00 late fee will be added to your account. You may pay by cash, check or through the parent portal. You may also sign up for auto-pay.

Attendance
If you are unable to attend class please contact us at indianadancecompany@gmail.com prior to class time so we can plan accordingly for the absence. There are no refunds on tuition for missed classes. If a class is cancelled by an instructor, a makeup class will be scheduled to make up the time missed.

Studio Closings
In cases of inclement weather, we will follow RBB closings. The studio will be closed on all major holidays: Easter, Memorial Day, July 4th, Labor Day, Thanksgiving, Christmas Day, Christmas Eve, New Years Eve, New Year's Day.
