I/we agree to release and hold harmless Miss. Annabelle's Dance (MAD) including its teachers, dancers, staff members, and facilities used by from any cause of action, claims or demands now and in the future. I/we will not hold Miss. Annabelle's dance (MAD) liable for any personal injury or any personal property damage, which may occur on the premises before, during, or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Miss. Annabelle's Dance (MAD)

I understand that Miss. Annabelle's Dance (MAD) is licensed, accredited, and insured organizations. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Executive Director, Artistic Director, instructor or staff member as soon as possible.


PHOTOGRAPHY/ VIDEO – I give permission for my son/daughter’s photographs/videos to be used for publicity and advertising in the studio, on the M.A.D STDIOli website, and in the community.

Assumption of the Risk and Waiver of Liability
Relating to Coronavirus/COVID-19

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Miss. Annabelle’s Dance (MAD Studio) has put in place preventative measures to reduce the spread of COVID-19; however, MAD Studio cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending MAD Studio could increase your risk and your child(ren)’s a risk of contracting COVID-19.

By agreeing/ checking the box to this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child(ren) and I may be exposed to or infected by COVID-19 by attending MAD Studio and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at MAD Studio may result from the actions, omissions, or negligence of myself and others, including, but not limited to, MAD Studio employees, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my child(ren)’s attendance at the MAD Studio or participation in MAD Studio programming (“Claims”). On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless the MAD Studio, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of MAD Studio its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any MAD Studio program.

Please understand that I do not work 24/7. Any messages after 9:00 pm on weekdays or on weekends Saturday-Sunday will be answered on the Upcoming Monday Morning (unless it's a holiday)

Monthly Tuition:
Tuition is due on the first class of the month. $15 tuition FEE will be billed if payment is 2 weeks late. Please take into account that Running a dance studio is expensive. Payment must be in ON TIME to keep your dancers spot in the class and to keep the business running. Some classes have a waitlist, if payments are persistently late the dancer will get bumped. If a check gets Bounced my bank charges me a $33 bounce fee. The parent or guardian will be expected to pay this fee. Tuition rates will remain consistent regardless of the number of lessons/weeks in a month as monthly variations have been taken into account when determining the class price. All payments must be up to date and balance paid in full in order for a student to receive apparel or merchandise, or before participating in class, any special event, showcase, recital, or competition.

Weather Cancellations:

All snow closures will be an announcement as far in advance as possible. If Schools are closed or after school activities for inclement weather, Miss Annabelle’s Dance will also be closed. This is for the
safety of the dancers and Drivers. Should you have questions as to whether or not dance class is canceled, feel free to contact me. There will be no
make up classes for days missed due to weather.

Cancellations:
If the instructor needs to cancel a class for personal reasons, information for makeup classes will be posted.

Missed Classes:
If a student will miss a class we request that you let the instructor know 24 hours in advance. Students participating in the recital should make all reasonable efforts to attend as many classes as possible once recital choreography has begun. This is in the student’s best interest so as not to fall too far behind in learning routines. Private lessons (at prevailing rates) are available to those who have fallen behind in learning the choreography. If a dancer misses an excessive amount of classes leading up to the recital dancer may or may not be able to participate in the recital, due to the safety of other dancers.

Front Desk and waiting room Expectations:
Please be considerate of other customers as well as our students while in the studio or waiting room areas of the studio. The studio and the front desk are one-room please be quiet and respectful when entering the building. Just as you can hear some of the instructions coming from inside the studio, dancers and instructors inside the studio can hear loud noises from the lobby and waiting area. Siblings of students need to be supervised at all times within the property of Miss. Annabelle’s Dance Studio. There should be no running, yelling, or screaming! The waiting room is fully equipped with benches, couches, plugs sockets, wifi, Dance books, tea, coffee, hot chocolate, water, and Gatorade. This is to keep my parents and students satisfied

Parent Observation:
Parents are NOT permitted to watch classes unless invited by the instructor. Please remember
that this is as a courtesy to your dancer and all other dancers in the studio. Their dance education is of utmost importance to me. I understand that adjusting to the first few classes may be difficult for some of our creative movement/beginners dancers. We permit parents to join students for the first few classes until dancers warm up to their teachers, peers, and surroundings. Parents are not permitted in the creative movement/beginners class after October 1st.

Student Evaluations & Level Information:
Miss.Annabelle’s Dance reserve the right to place students at the appropriate level. Students age 6 and up will be placed in levels based on technical ability and maturity, not their age. Students should plan on spending at least two years at a level before continuing on to the next level. Miss. Annabelle is more than happy to provide additional feedback and help students develop and succeed in their dance goals. When fall classes begin and I feel that your student has improved past our spring expectations, I will contact you to reassess class levels.

Classroom Expectations:
Students should be ready to enter class on time, have used the restroom, with hair up, and in the appropriate dress code for the class, they are preparing to enter.

Dress Code:
The Miss.Annabelle’s Dance dress code was created to keep your dancer safe, prevent injuries, and ensure that he or she receives the best dance instruction possible. A copy of our dress code is included on the last page of this packet. Additional copies of our dress code are available at the studio front desk.

Behavior:
Students and all accompanying persons (parents/siblings, etc) will practice respect to teachers, classmates, visitors, and themselves. Any students, family members, visitors, etc who act in a disrespectful or disruptive manner may be required to have a meeting with the instructor and administrative personnel to discuss the issue. After multiple infractions, or in rare severe single incidents, the student and family may be asked to leave the facility permanently and will not be offered a refund or proration. Students representing Miss Annabelle’s Dance (at events outside the studio facility as well as when wearing any apparel that denotes Miss Annabelle’s Dance logo/name) will maintain the same level of decorum and mindful behavior listed above.

Safety Policy:
We request that all students age 7 and under be escorted to and from the studio by an adult. The instructor arrives 15 minutes prior to the first class of the day. Please do not drop off students until the teacher is present. Do not leave students unsupervised either inside or outside the facility.

Drug and Alcohol:
Possession of drugs and/or alcohol: Any student found using drugs, or consuming or under the influence of alcohol
or show up to classes or rehearsal Hungover (including providing drugs or alcohol to another student) will face expulsion.
Release of Liability:

I acknowledge and consent to allow Miss. Annabelle's Dance (M.A.D STUDIO) to use:
photos and/or videos of my child/myself in publications (such as flyers/brochures),
advertisements (such as newspapers or online ads), our studio’s website, or on social
media sites (such as the studio’s Facebook or Instagram account). I understand that in
some cases, my child and/or my name may be used. I further understand no financial
compensation will be for use of these photos/videos

Miss Annabelle’s Dance shall be released from any and all liability claims, demands and causes of action whatsoever, arising out of or related to any loss, damage or injury that may be sustained by the participant and/or any other individuals while in or upon the premises or any premises under the control and supervision of Miss Annabelle’s Dance, or in route to or from any of said premises
