You must be over 18 to register for an account, and a checkbox label of "I am over 18".
Dancers must wear a cover up over their dance clothes and street shoes when entering and exiting the building. BALLET SLIPPERS MAY NOT BE WORN IN THE PARKING LOT.
Tights under Leotards serve as undergarments. So, no visible undergarments or underwear should be worn under tights.
Hair must be pulled neatly into a bun or twist for ALL classes, ALL levels. All bangs and wispies should be pinned or secured up away from the face and neck. Ladies can wear small, tasteful hair decorations and small earrings. No other jewelry may be worn.
Gentlemen need to keep their hair trimmed so it is off of their face and neck.
Shoes must fit snuggly! Ballet shoes and pointe shoes cannot be worn with “room to grow.” Do not sew any new pair of shoes until the fit has been approved by an instructor. When dancers are approved to begin the study of pointe work, an instructor will accompany them to their first pointe shoe fitting.
Creative Movement and Pre Ballet
Ladies: Any style plain black leotard, pink ballet tights, pink ballet slippers.
Gentlemen: black or white t-shirt, black or white shorts or leggings, black or white ballet slippers. Socks optional.
Ladies: plain black leotard, pink ballet tights, and pink ballet slippers. For some productions, tights with seams will be required.
Gentlemen: young dancers may wear white t-shirt, black shorts, black or white ballet slippers with matching thin socks.
Intermediate and Advanced dancers must wear white t-shirt, black tights, dance belt, black ballet slippers.
Levels 5/6/7 dancers may wear a solid color leotard of any color on Saturdays. Gentlemen may wear a solid color t-shirt of any color on Saturdays.
Comfortable work-out clothes, ballet slippers.
Warm Up Attire
We recommend the dancers arrive early to spend some time warming up before class begins and wear long sleeve leotards during the winter months.
If the studio is cold and the instructor gives permission, dancers may wear snug fitting sweaters. Levels 5 and up may also wear warm up tights or leg warmers. During Contemporary classes dancers may wear jazz pants. All warm ups must be solid black or grey colors only. The teacher must still be able to see if the dancer is in correct alignment, so no loose or bulky clothing, bright colors, or distracting patterns will be allowed. It is up to the individual instructor to decide what is acceptable as well as when the warm ups must be removed during the course of the class.

WAIVER AND RELEASE FROM LIABILITY
I, as the undersigned person, parent or legal Guardian has chosen to participate in, or have their child participate in, dance instruction given by The School of Ballet Indiana, LLC (the “Company”), and the undersigned expressly gives permission for such dance instruction. I acknowledge and understand the nature of the activities that I or my child will be participating in and that I am or my child is in proper physical condition and capable of participating in the related activities, understanding that the Company is not in any way responsible for making such a determination.
I understand and agree on behalf of myself and if applicable, also my child, to RELEASE and DEFEND, INDEMNIFY, AND HOLD HARMLESS the Company, its owners, instructors, employees, independent contractors, landlord(s), volunteers, assistants, or other students from all claims, actions, demands, costs, liabilities, expenses or judgements, whether arising out of the Company’s negligence or not and whether foreseeable or unforeseeable, including but not limited to attorneys’ fees and court costs, for any occurrences arising out of or related to dance instruction or any other activities conducted by the Company or at the Company’s place of business.
I understand that the Company is not responsible for my child or other children under my supervision who are left unsupervised in the common areas and areas surrounding the studio and that the Company will only be supervising my child when he or she is participating in scheduled activities, programs, and instruction. Nothing contained in this paragraph shall affect the agreement set forth in the immediately preceding paragraph.
I understand that the Company is not responsible for personal property that is lost, damaged, or stolen while I am or my child is at or in the Company’s place of business.
I acknowledge and agree that it is my responsibility to maintain my own accident and health insurance coverage providing adequate coverage for myself and/or my child participating in Company activities and that the Company does not provide accident or health insurance for those participating in its instruction, activities, or programs. In the event that the parent/contact cannot be reached in an emergency, I give permission to the Company to render First Aid, as well as being taken to an emergency room or to call an ambulance to secure medical treatment, as needed.
I authorize and agree that the Company may take and use photographs, videos or likenesses of myself or my child for its record-keeping, advertising and/or public relations projects and that I have no rights to the same and will not be compensated for the same.
The Checked Box is proof of my intention to execute a complete and unconditional waiver and release of all liability pursuant to the terms herein. I am of lawful age and competent to sign this affirmation. If I am signing for my children, I certify that I am the parent or legal guardian and have right to waive these rights.
SBI encourages everyone to attend classes as much as possible. For the times that classes are missed, please be aware of the make-up policy which applies to all sessions.
A missed class can only be made up in a lower level and in the same month (week for June/Back to the Barre Session) as the missed class.
*Example-a level 7 dancer misses 1 class in September. That dancer can make up class in any level 1-6 class in September.
Thank you for choosing SBI. Tuition at SBI is non-refundable except in cases of extended illness or injury beyond 4 weeks. At that time, arrangements will be made to credit/refund tuition minus any discounts. Registration fees are non-refundable.
SBI is so proud of our boys! We believe that boys are to be treasured and supported in the art of ballet. For this reason, SBI proudly offers boys, that enroll in the Fall/Spring Session, a 50% discount off of the tuition rate. Summer Session, Private Lessons, Master workshops, etc. will be at the normal rate.
