I understand that dance classes may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. Creative Connexions Performing Arts is not responsible for personal property.

I have received the student handbook and agree to adhere to all the content stated therein including: Studio Policies,Tuition & Payment Information, Dress Code, Traffic Pattern, Visitor Weeks and Calendar

I agree to be responsible for reading studio correspondence and respecting deadlines, if applicable.

In allowing my child to participate in Creative Connexions Performing Arts Centre's activities, I hereby assume all the risks associated with the performing arts. I understand the importance of myself and my child following the instructions and rules set by their instructor/s, and I agree to release Creative Connexions Performing Arts Centre and it's employees of any and all liability which may arise as a result of my child's participation in activities at Creative Connexions Performing Arts Centre.

The undersigned gives permission to Creative Connexions Performing Arts Centre, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.

Our Payment Policy:
Creative Connexions collects payments by postdated cheques, pre-authorized credit cards, EMT or cash payments at the time of registration. Every family will have a Dance Studio Pro account to view active invoices. To cancel any registration written notice must be given 2 weeks prior to the start of the term and $15.00 withdrawal fee must be paid. Upon cancellation, you are obligated to pay for the present pay period, all further payments will be void. There is no refund for membership fees anytime and the costume deposit after December 1st. All families are required to have a credit card on file, this card will never be charged without your notice. A $15.00 fee will be charged when an account has an outstanding balance 5 days past the due date, at this time your credit card on file will be charged. late fees are charged to any outstanding balance, not just class tuition. All returned cheques have an N.S.F service charge or $45.00.
