Agreement for Participation:
I understand that dance classes and rehearsals may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all the activities of dance class and rehearsal involve some degree of risk, strain, or bodily injury. MYBT School is not responsible for personal property or any type of injury.
I agree to be responsible for reading studio correspondence and respecting deadlines, if applicable.
I hereby acknowledge that I have read the statements above and agree to participate accordingly.

Photo Release Form for Adult and Minors

Michigan Youth Ballet Theatre School has my permission to use my or my child’s photograph publically to promote MYBT School. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
