Liability Release I on behalf of and as the legal guardian of my son/daughter agree to release from responsibility Clarke Dance Productions, including all staff, contractors, and teachers, from any liability for any personal injury, illness or property damage and loss that may occur on the premises or at company related events. Despite precautions being taken by Clarke Dance Productions, dance is a physical activity and accidents and injuries may occur. By signing below, I realize that participation in such activities could result in possible personal injury to myself or my child. I accept all risks related to the use of any and all spaces used by Clarke Dance Productions. Photography/Video Release From time to time we may post videos or photos of class activities and participants to social media and/or our website. Our recital is also captured by a professional videographer and distributed to registered families as a keepsake. By accepting this waiver you understand and consent to photos and videos being taken during class activities and that these photos may be posted online. If you would like to opt out of public photos, you must do so through the parent portal under more > Waiver > Photo Opt out and we will refrain from using images of your child in any social media post, website photos, or advertising. Participation Agreement I have read Clarke Dance Productions policies and I agree to abide by all rules and regulations set by Clarke Dance Productions, and to ensure that my son/daughter also follows these set guidelines. I agree to pay for all fees associated with my son/daughters participation in classes and events organized by Clarke Dance Productions in a timely manner. I understand that all NSF cheques will be subject to a $50 fee added to my account. I understand that all payments that are five or more days late are subject to a $25 late fee. Should I choose to withdraw from any class, I will only be refunded money pertaining to any installment periods not yet entered into. I understand that there will be no refunds for payments made for classes during an installment period and that I must withdraw prior to the start of the next installment period. I understand that there will be no refunds given for classes cancelled due to inclement weather.
