I, the undersigned, parent or guardian, do hereby agree to allow my child to participate in Pennsylvania Academy of Dance Arts (PADA) classes and other related activities and I further agree to indemnify and hold harmless PADA, and its directors, instructors and other unnamed assistants, from and against any, and all liability. This release also applies to myself, guests and my other children who are on the premises of Pennsylvania Academy of Dance Arts, or a participant in an off-facility location.

We often take pictures of children participating in activities or use group photos to be used for program advertisements and marketing. I grant permission to Pennsylvania Academy of Dance Arts to take photographs and grant use of such purposes stated above.

Pennsylvania Academy of Dance Arts (PADA) is doing everything possible to make it safe for all of our dancers and families. While enhanced safety measures & precautions are being taken, an inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is an extremely contagious disease that can lead to death. According to the Center for Disease Control & Prevention, senior citizens & guests with underlying medical conditions are especially vulnerable. By coming to PADA you voluntarily assume all risks related to exposure to COVID-19.

I agree to be responsible to Pennsylvania Academy of Dance Arts (PADA) for all tuition and fees for the students enrolled under this account. Tuition will continue to be charged until PADA is given 2 weeks written notice of the dancer's withdrawal from class. I understand that there are no refunds or credits issued for classes missed due to holidays, vacation, weather, illness, etc. Make-up classes are available. Tuition payments and all associated fees are non-refundable.

I understand that dance is a physical activity and involves some degree of risk of bodily injury. I herby release Pennsylvania Academy of Dance Arts (PADA), its owners, directors, teachers and staff from any liability of accident or injury occurring on or around studio premises and/or at any event/location related to studio activities. I declare the student(s) in my Family Account to be in good health and able to fully participate in the enrolled classes. In the event of injury or emergency, if I am unable to be contacted, I give permission for Pennsylvania Academy of Dance Arts to obtain medical services for this student.

You are registering for an in person class, however, in the event of a local, state or federally mandated lockdown/stay at home order due to a pandemic or other (natural disaster/alien invasion/zombie apocalypse/etc) Pennsylvania Academy of Dance Arts (PADA) will run classes live via Zoom (or other streaming technology) and there will be no refunds or tuition prorating.

PADA often takes pictures/videos of our students participating in classes and activities to be used for advertisement and marketing purposes. I grant permission to Pennsylvania Academy of Dance Arts to take such photographs and grant permission to use my child's picture/image on the PADA website, Facebook page, Instagram page or other social media and print outlets.

In the event our classes must move to a virtual platform, I acknowledge and agree to the following:

I hereby affirm and acknowledge that I am giving full permission for the above named person(s) to participate in a virtual learning dance program offered by Pennsylvania Academy of Dance Arts, LLC if that option is deemed necessary due to unforeseen circumstances by either the staff, an act of God (i.e. tornado, flooding, blizzard, etc.), or national/international crisis. I acknowledge that I am taking on all risk associated with the participation in an activity taking place outside of the brick and mortar establishment of Pennsylvania Academy of Dance Arts, LLC. I understand that it is my responsibility to provide a safe, conducive, and (reasonably) open environment for each class that the above-named person(s) will participate in virtually. In consideration of myself, or the above named person’s participation in the virtual dance program, I, for myself, my heirs and assigns, hereby release Pennsylvania Academy of Dance Arts, LLC, the owner/director Pamela Sterner Boyer, director Kimberly Landle, their families, and/or employees/instructors from any and all future rights, claims, causes of action, civil or criminal claims, demands, costs, attorney fees, loss of service, expenses, compensation, third party actions, suits of law or equity (including suits for contribution and indemnity), of whatever nature, and all consequential damages on account of, or in any way associated with the Pennsylvania Academy of Dance Arts, LLC.

I also understand that by agreeing to these terms, there will be no refunds, pro-rations, or credits given for any reason; nor will an account be suspended until such time that virtual learning is deemed unnecessary.

By agreeing to these terms, I hereby affirm that I have read and fully understand the foregoing release and know the contacts thereof. I acknowledge that agreement by either parent or by one guardian hereby binds all parents and/or guardians of any minor participant.
