I authorize the merchant known as Carlson's School of Dance to charge my credit card for purchases related to festival fees, competition fees and extra costume fees. Carlson's will provide email before charging my card on file.

*Credit Card Authorization Form will be e-mailed to you or you can come by the office to fill one out.

Carlson's School of Dance shall not be liable for any accident or injury of whatever nature or sort, whether resulting directly or indirectly from any activities on our premises, or otherwise, regardless of cause, unless due to gross negligence on the part of Carlson's School of Dance and shall not be liable for any loss or damage, whether by theft or otherwise, to any articles belonging to any person brought onto our premises. All Studio Policies are listed in the Student Handbook on our website. Photos/Videos taken at performances and in class may be used by Carlson's School of Dance for the purpose of any advertising and promotion.

Withdrawal from classes requires one month's written notice (e-mail is acceptable). Failure to notify the office will result in a charge equal to one month's payment. Withdrawing halfway through the month will require that month's payment.

After April 1st we have a no withdraw, no refund policy, and final payments for the year are due. If you have to withdraw, payment in full is required for the remaining months.
