The student hereby gives permission to Nicole Rocks School of Dance, its employees, and its agents to release and use reproductions of students likeness in photo, print, video or other format for the promotion of the activities of Nicole Rocks School of Dance in any media including Nicole Rocks School of Dance Website, Facebook page, or newspaper.
In the event that the student requires emergency medical attention, student hereby gives consent to Nicole Rock or staff, on behalf of Nicole Rocks School of Dance, to make necessary decisions for the provision of medical care to the student in the absence of an ability to reach the students parent, if under eighteen, or the students emergency contact.
Student understands and recognizes that there are inherent risks involved in any physically demanding activity like dance, acrobatics, tumbling or other similar activity as well as risk of covid or other sickness. Student does hereby waive, release, and hold harmless Nicole Rocks School of Dance and its employees, teachers, sponsors, and volunteers from any and all liability or claims of right for damages from injuries growing out of, related to, or arising from participation in Nicole Rocks School of Dances activities.
Student further certifies that, having full knowledge of the risks involved in the planned activities, student is physically fit to participate.
Student will pay one month in advance for class(es), and if payment is not made timely, student will be notified. If payment is not made after notification, student will no longer be able to participate in class. Payments 15 days overdue will incur a late fee of $10. Payment for pictures, costumes, shoes, or other items are not covered by the class fee. If for any reason your student drops our 2024-2025 season classes after Feb 1, 2025 payment is due in full for remainder of months left for the season. If you choose to use online credit card services you will be charged a convenience fee. You may choose to pay by ACH thru NRSD or you may also pay by check or cash at the dance studio. There is a $30 return check fee on any returned checks. We have taken our into account our scheduled breaks and time for lesson planning and choreography and divided that over the dance season. The dance fee remains the same no matter if we have 3 or 5 weeks of classes that month.
Covid-19 Notice
Nicole Rock's School of Dance will not be held liable for any students, parents or anyone entering our facilities who may become sick/ill from covid-19. We will take all precautions we can to prevent illness. We will not refund anyone who chooses not to attend due to covid-19 or other illness.
You will receive $6 credit per item sold towards your costume account from our fall Little Lambs Fundraiser if you choose to sell. This will go into your childs account and is not redeemable for cash. If you leave the program for any reason prior to recital the fundraiser money or costume is non refundable to you. If your account exceeds the costume fee you will have a credit to use on lesson payments.. If you do not sell enough items to pay for your costume fees you will owe the costume balance by November 21, 2024. The costume fee is non refundable. If you leave the studio for any reason prior to recital costumes become the property of Nicole Rocks School of Dance IF earned with fundraising money and will not be given to student if they leave the program before the recital if earned by fundraising. Students who complete the program will keep their costume and anyone who paid out of pocket for their costume and drops classes may contact the studio for that costume. They will not be held for you past June 15,2025. Any funds not used by June 15, 2025 from the fundraiser will transfer to Nicole Rocks School of Dance, as credit is only good for one studio season.

All costume fees are subject to sales tax of 7% on top of the listed fee above. The fees do include one pair of recital tights per dancer if needed for recital. Competition students are required to have studio specific brand/color of shoes for each class.
Any unpaid costume fees past the deadline will result in your students costume not being ordered. There is not a guarantee that we will be able to get your costume in on time. There will be an additional ordering fee for the costume if it is ordered late.

Family discount $75 per month for all like classes within one household at one studio
West Liberty combo discount- 1/2 off the 3rd class at this location least $ class/does not include competition
WLF- 10% with yearly lessons. Current photo yearly of badge must be turned into studio to receive discount

Dress Code:
All students of NRSD are expected to have the correct dance shoes required for each class. No jeans, dangle earrings, or jewelry allowed. All girls must have hair pulled up. Dance/active attire are acceptable. Please do not wear school clothes to dance. NO CELL PHONES OR GUM ALLOWED IN CLASS.
CLASS TIME:
Being in class is important. This is the only way students will learn. We will have an attendance program and students with perfect attendance for the year will receive a trophy! WE ALLOW ONLY ONE MAKE UP CLASS IF YOU MISS YOUR SCHEDULED CLASS. We strongly encourage students to be in class every week! If your student will miss class please send us an email or call to let us know they will be gone.
TOT DANCERS:
The little students who are just becoming potty trained may want to wear a pull up to class to prevent any accidents. We do have helpers who will assist them in the bathroom.
WAITING AREA:
Please be respectful in the waiting area. Do not climb in the windows or jump from chairs. Clean up your trash and students should sit quietly while waiting.
Before and after class students may use the waiting room. There is not supervision in the waiting room and it is the parents responsibility to be there on time to pick up the student. We do have star company helpers who will be monitoring pick up and drop off of young students when they are available. We ask that parents wait outside the building. If you have a young child and would like to wait with them please wait outside together and then we will come to the door when we are ready for their class.
We will ask that parents/caregivers wait outside our building for drop off and pick ups.
CLOSING:
In the case of inclement weather we will notify you by the email you provide at enrollment. The closing will be posted on Facebook under: Nicole Rock's School of Dance. We will honor the school closings in the town of each studio. Our students safety is important. We will make the best judgement based on the incoming weather conditions, considering if we are in a winter weather warning.
A general make up online class or video will be offered in the case of a closing due to weather or staffing.
Competition Team:
Students who audition and are accepted onto competition teams are required to attend scheduled competitions with the teams. Anyone missing more than 2 scheduled practices can be dropped from the team. All fees for competition and costume will be posted to your account and due upon receipt.
Competition solo/duets:
Students are required to keep their competition schedule and follow rules in the competition agreement. This form will be given to each student and must be returned prior to any practices taking place.
