Any payment not received at the beginning of the first class of the month will be charged a $5.00 late fee for each week that the payment is late. The tuition payment and any fees accrued will be due at the beginning of the next class. Beginning September 1, 2022, any account delinquent in payment for more than 60 days will be required to enroll in automatic electronic payment in order for your dancer to continue classes. In addition, any returned check with have a $25 returned check fee. Tuition is due the first week of each month September- June beginning the week of September 6, 2022 ** tuition is based on total number of classes per year, not per month** All tuition must be current to participate in performances. A total of 3 inclement weather/emergency days are built into our schedule. Any classes canceled after those days are exceeded will be made up at a date and time announced by the individual instructor. All charges including registration, costuming/performance uniforms and tuition is non-refundable.
Tuition:
Welcome to Expressions Performing Arts Center. Out tuition is broken up into monthly installment payments payable Sept- June, or may be paid upfront in one sum at the beginning of the year. Tuition is based on total classes per year, not per month.
Tumbling- $60 per monthly payment
Tiny Dancer Tap/Jazz/Ballet combo- $ 60 per monthly payment
All recreational dance classes- $60 per monthly payment
EPAC Revolution Dance Company competition team members- each class- $54 per monthly payment
Recital Costumes will be priced individually by class at $60 per class. All costume payments are due in full no later than November 1, 2022
2022-2023 Recital Dates TBA. All stdents will participate in our December Holiday Show and our Annual Spring Show in June
Tuition is due in monthly payments September- June. All paid registration, costume, and tuition fees are non refundable.
I acknowledge that I understand the late fee and payment policies and will cooperate with Expressions Performing Arts Center and abide by the policies.
I understand that dance and fitness activities do require extensive physical activity and there is always a chance of injury. I do not hold Expressions Performing Arts Center responsible for any injury or incident the above named student may sustain associated with the dance, tumbling or fitness program or with any activities associated with Expressions Performing Arts Center, its instructors and/or contractors.
I allow my dancer's photo and image to be present in any media regarding Expressions Performing Arts Center (EPAC). This includes, but is not limited to, press releases about shows and studio events and dance studio photos in print, tv, dvd and radio promotion, on the EPAC website, Facebook or other social media.
I HEREBY ASSUME ALL OF THE RISKS OF PARTICIPATING IN THIS ACTIVITY OR EVENT, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.
I certify that I am physically fit, have sufficiently prepared or trained for participation in the activity or event, and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity or event.
I acknowledge that this Accident Waiver and Release of Liability Form will be used by the event holders, sponsors, and organizers of the activity or event in which I may participate, and that it will govern my actions and responsibilities at said activity or event.
In consideration of my application and permitting me to participate in this event, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:
I HEREBY WAIVE, RELEASE, AND DISCHARGE Expressions Performing Arts Center and all divisions thereof of any and all liability and responsibility for injuries, sickness, pandemics, accidents, natural disasters and/or acts of God incurred during participation in and/or instruction of camps, intensives, private instruction, choreography, classes or any activity I may participate.
I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this event, THE FOLLOWING ENTITIES OR PERSONS: Expressions Performing Arts Center and/or their directors, officers, managers, employees, contractors, volunteers, representatives, and agents, the activity or event holders, activity or event sponsors, activity or event volunteers;
I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this waiver, release and registration form from any and all liabilities or claims made as a result of participation in this activity or event, whether caused by the negligence of release or otherwise.
The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
