I acknowledge that costumes are a part of the Spring Recital performance and chosen by Revolution School of Dance. I understand that all costume fees are NON-REFUNDABLE and no refunds will be issued under any circumstances. If I choose to drop classes before costume pick-up, I will lose the costume fee. Once a costume order has been places and payment has been made, no refunds will be issued and no costume will be available for pick up.

I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Revolution School of Dance, its officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “Revolution School of Dance”).

I hereby agree to release Revolution School of Dance and hold Revolution School of Dance harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.

I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.

If I am a minor, my parent and/or legal guardian has also signed this document releasing Revolution School of Dance from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.

The participant has my permission to participate in Revolution School of Dance Events. I warrant the below information is complete and correct. I further release Revolution School of Dance of all liabilities associated with my child’s attendance at Revolution School of Dance.

I give Revolution School of Dance permission to use my child’s picture in or on any form of advertisement for Revolution School of Dance or a Revolution School of Dance affiliated event.

All dancers MUST have a credit card or bank account on file in order to enroll in the season classes. Monthly statements will be emailed on the 1st of every month showing charges and credits to your account. You can still make cash or check payments in the studio until the 10th of each month.

ALL ACCOUNTS NOT PAID IN FULL BY THE CLOSE OF BUSINESS ON THE 10TH WILL BE BILLED TO THE CREDIT CARD OR BANK ACCOUNT ON FILE FOR REMAINING BALANCE AND ARE SUBJECT TO A CONVENIENCE FEE OF 2.75%.

Full tuition/fees will be charged even if your child misses a class due to illness, vacation or other activities. This policy is in effect because costs for staff and programs remain the same even when some students are absent. Your monthly tuition is reserving your child's spot on our roster and in order to guarantee your spot, full tuition must be paid.

All tuition, deposits, studio fees (including competition entry fees for our company dancers) and costumes are non-refundable.
