CORONAVIRUS / COVID-19 WARNING, DISCLAIMER, AND CUSTOMER WARRANT
Coronavirus, COVID-19 is an extremely contagious virus that spreads easily through person-to-person contact. Federal and state authorities recommend social distancing as a means to prevent the spread of the virus. COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Participating in Sandy Stramonine School of Dance or accessing Legacy Dance Academy facilities could increase the risk of contracting COVID-19. Legacy Dance Academy is in no way warrants that COVID-19 infection will not occur through participation in Legacy Dance Academy programs or accessing Legacy Dance Academy facilities. The undersigned hereby agrees, represents, and warrants that neither the undersigned nor such participating children shall visit or utilize the facilities, services, and programs of Legacy Dance Academy if he or she (i) experiences symptoms of COVID-19, including, without limitation, fever, cough or shortness of breath, or (ii) has a suspected or diagnosed/confirmed case of COVID-19. The undersigned agrees to notify Legacy Dance Academy immediately if he or she believes that any of the foregoing access/use restrictions may apply. The undersigned acknowledges and assumes both the known and potential dangers of utilizing the facilities, services, and programs of Legacy Dance Academy and acknowledges that use thereof by the undersigned and/or such participating children may, despite Legacy Dance Academy reasonable efforts to mitigate such dangers, result in exposure to COVID-19, which could result in quarantine requirements, serious illness, disability, and/or death.

1) All precautions are taken to safeguard our dancers from any injuries, but occasionally injuries do occur because of the physical nature of dance. Understanding such, I the undersigned, accept the normal risk involved in participating in any dance classes.

2)I hereby assume all financial responsibility for the above student enrolled at Legacy Dance Academy. I further understand that I will be charged for all classes unless I have notified the Studio in writing of canceling my child’s classes. In the event it becomes necessary to refer this account for collection, I, the undersigned will be liable for all collection fees, including attorney fees, interest etc. I have read all information and accept it.

3)I acknowledge that I have reviewed the policies listed in both the studio handbook the reopening guidelines and am aware of studio policies and procedure.
