I hereby register at eMotion dance studio. I agree to pay tuition in full by the 1st of the month and no later than the 15th of the month, after which a $10.00 late fee will be applied. There are no credits or refunds for lessons missed on the part of the student, however make up classes may be scheduled with the front desk. In order for a class to be dropped, parents must stop by the studio or call before the first of the month and give notice. Otherwise tuition will be charged by the month until notice is given. I understand that registration and fees are nonrefundable. I authorize eMotion Dance to charge my credit card. I realize that I will be charged unless I notify the front desk to discontinue charges beforehand. I authorize eMotion Dance to use my credit card if at any time my payment is 30 days late.

I hereby release eMotion Dance studio and staff from all claims of damages or injury suffered by the above registered student in connection with or by their association with the studio, excepting only gross negligence, relative to actions of decisions that concern or affect my child. This includes heirs who may not act in my behalf. eMotion is not responsible for children not picked up 10 minutes after class, unless previous arrangements are made.

I authorize eMotion Dance to use photography/video of the individual dancer in promotional material.
