LIABILITY WAIVER
I the parent or legal guardian of the children listed in this registration form realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by and programs, activities, rehearsals or shows run by GIBSONS DANCE CENTRE also operating under the name Dance Works BC.
I/we agree to release and hold harmless GIBSONS DANCE CENTRE (Dance Works BC) including its teachers, dancers, staff members, volunteers and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold GIBSONS DANCE CENTRE (Dance Works BC) liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by GIBSONS DANCE CENTRE (Dance Works BC).
I/we agree that GIBSONS DANCE CENTRE (Dance Works BC) will be informed of any physical limitations my child has before starting any programs with GIBSONS DANCE CENTRE (Dance Works BC) and with the understanding that my child's doctor has approved participation in physical activities such as dance.

I also commit to inform GIBSONS DANCE CENTRE (Dance Works BC) and its staff of any physical limitations, or prior injuries that may affect the safe training of my child so that the instructors can modify training to meet my child's needs safely.

I commit to keeping my child's emergency contact information up to date in my portal account. I also commit to keep my child's medical information up to date and will inform the GDC office of any changes to (injuries, illness, allergies, treatments, etc) by emailing: gibsonsdancecentre@gmail.com
I understand that in the event of a medical emergency the staff of GIBSONS DANCE CENTRE (Dance Works BC) reserve the right to seek appropriate medical attention for my child.

I understand that GIBSONS DANCE CENTRE reserves the right to use studio related (example: in class, performances, at studio events). photography or video of my child for the purpose of studio promotion or performance (example: GDC Social Media, posters, website).

I understand that if I would like to refuse that images or video of my child are to be potentially shared in the above format I am to inform GDC in writing via email: gibsonsdancecentre@gmail.com and await confirmation from office staff to confirm receipt of the request.

I UNDERSTAND THAT I WILL RECIEVE A COMPLETED INVOICE FROM THE GDC OFFICE WITH MY FAMILIES TOTAL TUITION AND ANY APPLICABLE DISCOUNTS WITHIN 3 BUSINESS DAYS OF THIS REGISTRATION SUBMISSION.

I, the parent or guardian indicated by name in this registration form, understand that I am taking sole responsibility for the tuition, registration and costume fees associated with the classes once I have completed registration. I understand that any payment arrangements with other family members, including the other biological parent of said child who is not residing in the same residence as myself, is my sole responsibility to organize. I understand that Gibsons Dance Centre (Dance Works BC) is unable to bill anyone other than myself for fees associated with my child's class unless a FAMILY PAYMENT AGREEMENT FORM signed by any other party taking responsibility for payment is handed into Gibsons Dance Centre Office or appropriate court documents naming both parents responsible for payment can be produced. I understand that my registration will only be completed and confirmed once I have completed both STEP 1 (Payment Plan Agreement) and STEP 2 (Payment Method - ie: Auto Debit, Credit Card, etc) for the season with the GDC office via the online form (Payment Plan Options): https://www.gibsonsdance.com/payment-plan-options.html
