Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19:

I understand and am familiar with all Coronavirus/COVID-19 symptoms and agree to keep my child home if he or she or anyone in our household displays any symptoms. I understand that temperature checks will not be taken upon arrival to Rise Dance Center and agree to take my child's temperature before bringing him or her to the studio. I will inform Rise Dance Center and immediately discontinue classes if my child or a member of our household tests positive for COVID-19. Rise Dance Center has many preventative measures in place to decrease the spread of COVID-19. Despite this, there is still a risk of catching the virus. I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child and I may be exposed to or infected by COVID-19. Further, I hereby release Rise Dance Center of all liability if my child or anyone in my household is exposed to or tests positive for COVID-19.
