At Danceworks Dance Studio, we are following strict social distancing + cleaning protocol. As any other public place, we cannot guarantee that your child will not be exposed (whether at Danceworks Dance Studio or elsewhere or if exposed, that your child will not contract the disease. By signing below, you agree to waive any claims of liability against Danceworks Dance Studio and to hold Danceworks Dance Studio harmless if your child contracts COVID-19 despite our best efforts to prevent it. Additionally, I agree to notify Danceworks Dance Studio if my child or any member of our household contracts COVID-19 or has come come into contact with someone who tested positive for COVID-19. This action will help ensure the utmost safety of our dancers + staff.
Registration and Parent Portal I hereby understand that each student enrolled is required to pay an annual registration fee of $25 (1st student) and $15 (each additional student) for their registered session AND their first month's tuition to secure their space in class. If I fail to submit my student's registration fee and first month's tuition payment within 24 hours of my registration, the registration system will automatically drop my student from their enrolled classes. The registration fee is a one-time fee. If you register during the early bird time your registration fee will be waived. I hereby understand that I am responsible for logging into my parent portal account AT LEAST once per week to view my account balance, class schedule, studio correspondence, and to keep my contact and student information up to date.
I hereby understand that attendance in classes is mandatory for my child's continued progress. The studio observes the following holiday/break periods: Labor Day, Halloween, Thanksgiving, Christmas, New Year, MLK Jr., Spring Break, Memorial Day, and Fourth of July.. Classes that do not meet in observance of these holidays are not refunded, prorated, or made up. Information regarding closings due to other emergencies beyond the control of the management or inclement weather is provided via recording by dialing the studio phone line 302-286-1492, studio Facebook & Instagram pages, www.danceworksde.net. Classes that are otherwise cancelled will be rescheduled for a make up date.
A $20.00 late fee will be assessed on the 4th of each month. A 5% late fee will be added to unpaid tuition balances on the 10th of each month and will increase by 5% every subsequent 5 days until payment is made. Additionally, if tuition has not been paid by the 15th of each month, your student will be unable to attend their registered classes until payment is made. If you bring your student to class with an unpaid tuition balance, they will be seated in class and not allowed to participate. There is no guarantee that your student's space in their enrolled classes will still be available after the 15th. You are still responsible for the full month's tuition and late fees in which your student was enrolled before your return.
I give my permission for Danceworks Dance Studio to take my child to a medical/dental facility, if necessary, in case of emergency, if parents or emergency contact cannot be reached. DWDS will not be responsible for the cost of any medical care or treatments. I hereby waive all claims whatsoever in connection with such medical treatments.
I hereby understand that the studio has 2 annual dance performances held in December and June. Each student is required to participate. Performances are a reflection of your child's progress. Concert fees are assessed per student for each performance. This includes costumes and participation fees. Annual end of the year performance has a recital fee of $175 for 1 dancer and $220 for 2 dancers. This fee is due by May 1st.
I give my consent for images (photographs, & video) of my child to be taken and used to document the activities of Danceworks Dance Studio. I grant DWDS permission to use images for educational and promotional purposes. I understand that if I DO NOT want images of my child used, I will indicate this in writing and the signed letter will be emailed or given to studio in a physical form.
I hereby understand that there are absolutely NO REFUNDS given for tuition, costumes, concert fees or registration fees.
I hereby understand that tuition is due by 11:59 PM on the 25th of previous month (regardless of the day). Tuition is based on 10 months (September - June) and upon the number of classes in which a student is enrolled. I hereby understand that I will not receive monthly billing statements or invoices and it is my responsibility to stay up to date with my tuition balances via the parent portal. Sibling Discount: 1st student = full class tuition price. Each additional sibling is 10% off each student's monthly tuition. To qualify, additional students must be siblings of primary student and live in same household. Paid In Full Discount: Full Year = 10% (September - June) ; Half Year = 5% (September - December; January - June)
I agree that if my child or I (heretofore known as "we") engage in any physical exercise, class, or activity on the premises or any venue where we participate as representatives of DWDS, we do so at our own risk. I agree that we are voluntarily participating in activities and use of said facilities, premises (including the parking lot) and designated DWDS venues. I assume all risk of injury, illness and damage, or loss to us or our property that might result, including, without limitation, any loss or theft of any personal property. I agree that this consent and assumption of risk statement covers each and every event or activity sponsored by Danceworks Dance Studio. I agree to release and discharge you (and your affiliates, employees, and agents) from any and all claims or cause of action (known or unknown) arising out of your negligence. I acknowledge that I have carefully read this Waiver and Release and fully understand that it is a release of liability.
