By registering, I understand and agree to the following:
Registration Fees are Non- Refundable.
I understand that I will be required to keep a credit card on file for automatic tuition payments. Payments are processed on the 5th of each month from the card on file (stored directly with the PCI-compliant payment process, not on file with our office). If I do not want the payment to be processed automatically, I must visit the box office before close of business on the 4th of the month during normal business hours.
ALL payments are DUE on the FIRST of each month. Please see above for the Auto-Pay Requirements. Late payments are $15.00 PER WEEK. Tuition is the same amount each month regardless if there are 3, 4, or 5 classes in a month. Those months with five classes will always make up for those that only have three (i.e. Thanksgiving, Christmas Break, Spring Break, etc.). Private Lessons function in the same manner as classes for all policies herein.
Class withdrawal requires a 30-day written notice. Please visit the box office during normal business hours to complete the request. Calls, texts, emails directly to instructors or AYT staff will not satisfy the written notice requirement. Tuition due or paid prior to any cancellation is non-refundable. PLEASE SEE BELOW FOR AYT COMPANY WITHDRAWAL POLICY.
We follow the scholastic calendar of the Higley/Chandler School District. We will not have class during Thanksgiving Break, Christmas Break or Spring Break. As for other holidays (Labor Day and Memorial Day) classes will be in session.
Enrollment and Instructors:
AYT reserves the right to cancel/merge classes as necessary to support the business. Communication will be provided if a class will be canceled/merged and tuition adjustments will be discussed on a case-by-case basis. AYT also reserves the right to replace an instructor at anytime.
AYT does not prorate, refund or credit tuition for missed classes. A student, however, is permitted to attend an equal number of missed classes within a 14 day period by coordinating a drop-in schedule with the box office.
Any applicable costume deposits and fees are non-refundable regardless of non-participation or withdrawal.
I, the undersigned agree to provide medical insurance for my Performer and will not hold Actor's Youth Theatre and Tuscany Theatre, or its agents or employees liable in the event of any accident or injury. If I am not reached in an emergency I give my permission to the staff to render aid or act on my behalf to obtain emergency medical treatment for my Performer for any injury that may occur while attending classes, lessons or rehearsals. Additionally, I give permission to AYT to use the registrant’s name and any photos or videos of the registrant for promotional use. I also understand that AYT is not liable or responsible for any lost, damaged, or stolen property during rehearsal, lesson or class time.
AUTHORIZATION TO TREAT PERFORMER: In the event that I, or other parent/guardian, cannot be reached in an emergency, I hereby give permission to secure proper treatment for my child. I do hereby consent to whatever x-ray, examination, anesthetic, medical, surgical or dental diagnosis or treatment and hospital care are considered necessary in the best judgment of the attending physician, surgeon or dentist and performed by or under the supervision of the medical staff of the hospital or facility furnishing medical or dental services. I further acknowledge that I have comprehensive health insurance coverage or am responsible for all medical coverage for performer in case of any emergency.
In addition to the above-mentioned policies, the additional polices below are enforced for AYT Company Members who have auditioned and agreed to participate for the season.
Company Dues are assessed at the beginning of each season but divided into 10 monthly payments for your convenience. Company Dues are non-refundable. Student withdrawal or non-participation WILL NOT CANCEL THE OBLIGATION TO PAY COMPANY DUES IN FULL and monthly billing WILL CONTINUE TO from my credit card on file as directed above.
Company rehearsals and events have mandatory attendance requirements that will be discussed at the Company Parent meeting at the beginning of the season. In most cases, conflicts require at least a 30-day notice. All conflicts must be submitted in writing to the box office and approved prior to the actual absence itself. Absences do not waive, cancel or negate any participation fees required for mandatory events. Company members are STILL RESPONSIBLE FOR ANY FEES ASSESSED THROUGH THE ENTIRE SEASON. REFUNDS WILL NOT BE ISSUED.
Actor’s Youth Theatre reserves the right to amend, update and/or restate this Agreement at anytime. The current policy in effect will be used to address any situations that arise, regardless of the original signature date.
