I have read and agree to the Bethany Ann Hellen Studios policies and dress code. I understand that with participation in this dance program, or any physical activity, there may be some risk of injury. I also understand that, while proper body alignment and safety during dancing is taught and reinforced throughout classes, there may be circumstances beyond the instructor's and/or studio's control. By checking the agreement box below, I will not hold Bethany Ann Hellen Studios, Inc., Bethany Basso or other Bethany Ann Hellen Studios instructors responsible for any injury my child or I may receive while participating in this dance program. I have informed Bethany Ann Hellen Studios of any and all previous injuries or medical conditions that the instructor needs to know in order to ensure my or my child's safety. I waive and release Bethany Ann Hellen Studios, Inc., Bethany Basso, Bethany Ann Hellen Studios instructors, heirs and their assigns from any and all rights and claims for injuries suffered or medical expenses which may occur as a result of the participation in this dance program.
