TUITION AND FEES: By signing this document, I understand that tuition is due by the 10th of each month, and that a $10 late fee will be charged if paid after the 15th. Tuition is charged by the month, no matter how many classes the student attends. I acknowledge that if my child withdraws from classes during the year, I must notify the Attitude Arts Academy staff thirty days prior to the date of withdraw, otherwise payment will be expected as usual. I understand that if my account is not paid in full within 60 days, my child will not be allowed to participate in class until further arrangements have been made. All classes are allowed two inclement weather cancellations which will not be made up.

PHOTO RELEASE: The Attitude Arts Academy is including photos of students, teachers, and studio activities on its website and printed materials. I understand that these pictures will be accessible to anyone with internet access. I give my permission for the Attitude Arts Academy to use photos of my child/children on its website, on Facebook, in printed materials, and in electronic forms of communication. I understand that if I do not want images of my child to be used, I will indicate this in writing and the signed letter will be attached to this document.

MEDICAL RELEASE: I give my permission for the Attitude Arts Academy to transport, or call an ambulance, to take my child to a medical/dental facility, if necessary. In case of emergency, if none of the emergency contacts respond, I hereby give my consent for emergency medical care to be prescribed by a duly licensed Doctor of Medicine or Doctor of Dentistry. This care may be given under whatever conditions are necessary to preserve the life, limb, or well-being of my dependent. I accept full responsibility for all costs of said medical care and any emergency treatments. The Attitude Arts Academy will not be held responsible for the cost of any medical or dental care, or emergency treatments. I hereby waive all claims whatsoever in connection with such medical treatments.

WAIVER AND RELEASE: I agree that if my dependent or I (heretofore known as "we") engage in any physical activity, class, or activity, or facility on the premises or any venue where we participate as a representative of the Attitude Arts Academy, we do so at our own risk. I agree that we are voluntarily participating in activities and use of said facilities, premises (including the parking lot) and designated Attitude Arts Academy venues. We assume all risk of injury, illness, damage, or loss to us or our personal property that might result, including, without limitation, any loss or theft of any personal property. I agree that this consent and assumption of risk statement covers each and every event or activity sponsored by Attitude Arts Academy. I agree to release and discharge you (and your affiliates, employees, assistants, volunteers, agents, representatives, successors, and assigns) from any and all claims or cause of action (known or unknown) arising out of your negligence. I acknowledge that I have carefully read this Waiver and Release and fully understand that it is a release of liability. I am waiving any right that I may have to bring legal action to assert a claim against you for negligence.

DISMISSAL FROM CLASS: The Attitude Arts Academy provides a fun, rewarding, and challenging experience in dance and performing arts to students of all ages and abilities. This results from the collaboration among parents, students, and teachers. Class participation, a positive approach to learning, adherence to policies, and parental support are important factors influencing the experience of your student and others enrolled in class. In order to maintain a welcoming atmosphere, the Attitude Arts Academy reserves the right to dismiss any student, as it sees fit.

COVID-19: I desire to participate, or allow my child/children to participate, in one or more voluntary extracurricular activities with the Attitude Arts Academy. The novel coronavirus known as COVID-19, has been declared as a worldwide pandemic, and is believed to be extremely contagious, and spread by person-to-person contact. Federal, state, and local agencies recommend social distancing, facial coverings, and other measures to prevent the spread of COVID-19. The Attitude Arts Academy will conduct certain classes, activities, and events, beginning in August 2020, and continuing into the 2020-2021 academic year. These activities, will be conducted with safety protocols appropriate under the circumstances at the time. For the safety of all people involved, participants will be required to adhere to all safety protocols, and are subject to immediate removal from any activity if they do not comply. Extracurricular activities are a privilege, and participation is voluntary, not required. In an effort to ensure the safety and wellness of our Academy, staff, and students, I understand the importance of students being healthy and safe when they participate in our programming, activities, and/or events. By electronically signing this document, I voluntarily assume all risks that I and/or my child/children may be exposed to or infected by COVID-19 as a result of participation, and that such exposure or infection may result in personal injury, illness, sickness, and/or death. In consideration of me and/or my children being able to participate in any program, activity, and/or event at or with the Attitude Arts Academy, I, on behalf of myself, as well as, anyone entitled to act on my behalf, hereby forever waive, release, and hold the Attitude Arts Academy, and its employees, and agents, harmless from any and all claims, suits, liability, actions, judgments, attorney fees, costs, and any expenses of any kind resulting from injuries or damages, grounded in tort or otherwise, that I and/or my child/children, or my or our representatives, sustain during or related to my or my child/children participation or involvement.
