Injury / Illness Waiver I acknowledge and agree that in participating in the "Wahoo Dance Academy", there is a possibility of illness and or injury to the dancer/student. I will not hold "Wahoo Dance Academy" or it's Owner/ Director responsible for any illness or injury that may occur while participating in the 'Wahoo Dance Academy. Photo Waiver "I hereby authorize Wahoo Dance Academy to publish photographs taken of my child(ren) and my child(ren)'s name, for use in the Wahoo Dance Academy's printed publications and website. I acknowledge that since my participation in publications and websites produced by Wahoo Dance Academy is voluntary, I will receive no financial compensation. I further agree that my child(ren)'s participation in any publication and website produced by Wahoo Dance Academy confers upon me no rights of ownership whatsoever. I release Wahoo Dance Academy, MLR Dance,LLC, its contractors and its employees from liability for any claims by me or any third party in connection with my child(ren)'s participation." Financial Responsibility By registering your child into classes at the Wahoo Dance Academy you have entered into an agreement with MLR Dance, LLC., dba Wahoo Dance Academy. You agree to pay tuition for classes by session due dates as established by the Wahoo Dance Academy. Tuition payments that are late shall be charged a $15.00 late fee. Any acoount that is 30 days past due shall be charged 29.00% interest on any unpaid balance monthly. Any Account that has an outstanding balance at the beginning of the next dance season shall not be eligibale to enroll students until such time as any outstanding balances are paid in full. I recognize that all returned checks will result in a $45 returned check fee. *In the unforeseen event that your child is no longer able to participate at WDA a written notice (not in email) must be given to the Director. Charges applied to your account up to the date that notice is received are the responsibility of the account holder.
