Agreement for Participation

I understand that dance classes may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. Chesco Dance Center is not responsible for personal property.

I agree to be responsible for reading studio correspondence and respecting deadlines, if applicable.

I agree to all tuition guidelines and have read the information provided detailing the payment information and number of payments.

I understand there are no refunds or credits given for missed classes or cancellations for any reason.

As with the transmission of any communicable disease like a cold or the flu, you may be exposed to COVID-19, also known as the “Coronavirus”, at any time or in any place.
Despite our careful attention to sterilization and disinfection, there is still a chance that you could be exposed to an illness in the studio, just as you might be at your gym, grocery store or favorite restaurant. Please know that participation in Chesco Dance Center’s classes and rehearsals is at your own risk.

I hereby acknowledge that I have read the statements above and agree to participate accordingly.

COVID - 19 Chesco Dance Center Participation Release
Thank you for your continued support and trust in Chesco Dance Center. As with the transmission of any communicable disease like a cold or the flu, you may be exposed to COVID-19, also known as the “Coronavirus”, at any time or in any place. Be assured that we have always followed state and federal regulations to limit transmission of all diseases in our office and continue to do so. Despite our careful attention to sterilization, disinfection, and use of personal barriers, there is still a chance that you could be exposed to an illness in the studio, just as you might be at your gym, grocery store or favorite restaurant. Social distancing has reduced the transmission of Coronavirus. Although we have taken several measures to provide social distancing and contactless participation, please know that participation in Chesco Dance Center’s classes and rehearsals is at your own risk. We ask that all families follow the local, state, and federal guidelines that are currently in place.
I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin HEREBY RELEASE AND HOLD HARMLESS Chesco Dance Center, their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Agreement for Participation


I understand that Gems Dance Company may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. Chesco Dance Center and the Gems Dance Company is not responsible for any injuries or personal property.
I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Chesco Dance Center, it’s officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities Hereinafter collectively referred to as “Chesco Dance Center” (CDC).
I hereby agree to release Chesco Dance Center and hold Chesco Dance Center harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.
I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
If I am a minor, my parent and / or legal guardian has also signed this document releasing Chesco Dance Center from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.
