In behalf of myself and my children, I understand that there are risks associated with, arising out of and inherent to the activities provided through Adrenaline Productions LLC, dba Adrenaline Dance and Performing Arts Studio. They include, but are not limited to, illness, injury and death. In recognition of this acknowledged risk, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Adrenaline Productions LLC DBA Adrenaline Dance and Performing Arts Studio, it’s officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as “Adrenaline Dance”.)

I hereby agree to release Adrenaline Dance and hold Adrenaline Dance harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks arising out of active participation in classes, performances and activities through Adrenaline Dance on behalf of myself, the legal guardian, and the participant/s.

I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the associated risks. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.

I also give Adrenaline Dance permission to use my child’s picture in or on any form of advertisement or publication in behalf of Adrenaline Dance or an Adrenaline Dance affiliated event. (Please notify us in writing if pictures cannot be publicly displayed for any reason)

I understand that I must remain on the premises for the duration of class for any of my children under the age of 14.

The participants for whom I am registering have my permission to participate in all Adrenaline Dance classes, performances and activities. I warrant the information is complete and correct and that I do have the authority to act and speak in behalf of the participants I enroll in classes at Adrenaline Dance. I further release Adrenaline Dance of all liabilities associated with my childrens attendance at Adrenaline Dance.

I acknowledge that I may OPT OUT of media sharing during registration by clicking the OPT OUT BUTTON or by notifying management in writing at any time. If I don't opt out, Adrenaline Dance and Performing Arts studio may use images and videos featuring my child/children on any platform for the purposes of sharing events, information, activities, instruction, advertising etc. Platforms Include, but are not limited to: Social Media, the studio website, Nextdoor, Google ads, local football programs, etc.

Tuition is due by the 1st of the month. *Auto-pay is required for enrollment. A $15.00 late fee will be added after the 15th if auto-pay is unsuccessful and/or a balance remains. Costume and Recital fees will be collected as instructed at the time of charge.

NO REFUNDS: Tuition, costume and recital fees are Non-Refundable and Non-Transferable once collected. You may withdraw from classes through your Parent Portal at anytime, but no refunds will be given for fees already paid, regardless of reason. No exceptions. You may continue to take classes through the end of the month in which you are paid in full. There are no refunds for missed classes, or pro-rates for holidays.

HOLIDAYS: We are closed on all Federal Holidays. Tuition will be charged in full regardless if there is a holiday closure. Tuition is figured using a 4 week basis. 5 week months offset any closures.

ILLNESS: No refunds will be given to missed classes due to illness, but you may attend an alternate class as a "make up" if desired. (We will be working hard on routines for the recital, so committing to and attending your own class is vital to everyone! In other words, don't make this a habit!)

PROLONGED ABSENCE: You may ask to have your account be put on a temporary hold if it will be a pro-longed absence. (3+ weeks)
