•All students must complete registration and emergency forms and pay class tuition before taking any classes at UDC Studio
•There is an annual registration fee (per dancer), which is due upon registration with tuition.
•All tuition is non-refundable, Exceptions are made with a doctors note stating medical reason necessary for withdrawal, or if a class is canceled due to low enrollment and a substitution is not available. 30 day written notice is required for any other student withdrawal.
•Our year is based a 37 week schedule - approximately 4 classes per month. Tuition price is based on a 37-week schedule and broken down to equal monthly payment amounts.

Upon submission of your registration form, you will automatically receive copies via email of the following:

1. UDC Studios Terms and Conditions of Acceptance Agreement.

2. Student/Parent/Legal Guardian Company Program Agreement (if registered in the Company Program).

3. Assumption of Risk, Waiver, Release and Indemnity Agreement.

Please retain these copies for your records. It is your responsibility to contact UDC Studios immediately via email if you have not received copies for your records.

BY AGREEING TO THE SELECTION, YOU ACKNOWLEDGE THAT THIS CONSTITUTES YOUR ELECTRONIC SIGNATURE.

COMPANY CLASSES

Acceptance into any of the UDC Studios Companies represents a commitment to oneself and also to the Company group itself.


Any withdrawals from the Company group, at any time, will result in a disruption to the Company group's choreography and team chemistry.

Upon acceptance to a Company group, full tuition is due for the September 2023 to June 2024 dance season. A Payment Plan option is available (see Tuition Class Fees section).

Student tuition will not be waived or refunded should a student refuse to participate for any reason, decide to withdraw, or be forced to withdraw from the Company before June 2024.

Should a student refuse to participate for any reason, decide to withdraw, or be forced to withdraw at any point during the season, all amounts paid to UDC Studios as Designated fees will not be refunded under any circumstances and any Designated fees not yet paid will become immediately due and payable. In addition, for each performance group a student is registered in, a re-choreography fee in the amount of $300 as applicable to each performance piece will become immediately due and payable.

There are no exceptions to this.

RECREATIONAL CLASSES

While we understand that students enrolled in these programs may not have the time commitment of a full time program, we also recognize that students enrolled in these classes are also committing themselves to a Recreational group.

Withdrawals from these classes are permitted until October 31, 2023, with the requirement that 15 days written notice be provided prior to October 31, 2023. If valid notice is given, then a pro-rated amount of tuition will be waived or refunded, as applicable.

After October 31, 2023, student tuition will no longer be waived or refunded for any of these classes, and full tuition will be due from September 2023 to June 2024.

Should a student refuse to participate for any reason, decide to withdraw, or be forced to withdraw at any point during the season, all amounts paid to UDC Studios as Designated fees will not be refunded under any circumstances and any Designated fees not yet paid will be become immediately due and payable.

There are no exceptions to this.

WITHDRAWAL BY UDC STUDIOS

We reserve the right to withdraw a student from a class or the studio if they:

Exhibit unsafe behaviour toward themselves, our students, faculty, staff and family members.

Use foul language, suggestions, or negative behaviour around our students, faculty, staff and family members.

Continually fail to follow directions from faculty or staff members.

Are disruptive around students, faculty, staff and family members.

Refuse to cooperate with the guidelines set forth by UDC Studios

If a student who is registered in a Company dance group is withdrawn from a class or the studio under these circumstances, full tuition remains due from September 2023 to June 2024. Student tuition will not be waived should a student be withdrawn from the Company group before June 2024. All amounts paid to UDC Studios as Designated fees will not be refunded under any circumstances and any Designated fees not yet paid will become immediately due and payable.

If a student who is registered in a Recreational class is withdrawn from a class or the studio under these circumstances full tuition remains due from September 2023 to June 2024. Student tuition will not be waived should a student be withdrawn before June 2024. All amounts paid to UDC Studios as Designated fees will not be refunded under any circumstances and any Designated fees not yet paid will become immediately due and payable.

UDC Studio is pleased to be able to conduct classes following the physical closure of the dance school due to COVID-19.

UDC Studio has conducted a thorough COVID-19 risk assessment and is implementing new processes in light of the recent pandemic. Should you wish to discuss any of these in detail, please email the office and time will be arranged to answer your questions.

In consideration of being allowed to participate in any way in any UDC Studio activities, I understand, acknowledge and agree to the following:

*I certify that I/my child is physically fit and able to participate in the program, event, or activity, and have not been advised otherwise by a qualified medical professional. I/they will not participate in any program, event, or activity in which I/they are not physically able.
*I agree to comply with any and all new rules, regulations, terms, and conditions for participation in the program, event, or activity. Including but not limited to the cleaning of my hands by means of handwashing with soap or hand sanitizer, remaining in my designated dance space, and not bringing unnecessary items to the studio.

*COVID-19: The coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization.
*COVID-19 is contagious and is believed to spread mainly from person-to-person contact. It is believed that an individual can be in infected with COVID-19 without their knowledge and be asymptomatic.

*UDC Studio has put in place preventative measures to reduce the spread of COVID-19. However, UDC Studio cannot guarantee that I or anyone else will not become infected with COVID-19, including my relatives.
Participation in a UDC Studio activity could increase the risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by participating in a UDC Studio activity, and that such exposure or infection may result in personal injury, illness, permanent disability, and death to myself or relatives. I understand that the risk of becoming exposed to or infected by COVID-19 at a UDC Studio activity may result from the actions, omissions, or negligence of myself or others, including, but not limited to, UDC Studio employees, volunteers, and program participants.

*I UNDERSTAND AND VOLUNTARILY ACCEPT AND ASSUME ALL the foregoing risks related to COVID-19 and accept sole responsibility for any injury or illness that may occur to me or my family. Further, I UNDERSTAND AND AGREE that this release includes any claims based on the actions, omissions, or negligence of UDC Studio, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any UDC Studio program(s), related event, or activity.

*If I or anyone in my household presents COVID-19 symptoms, or I am contacted by the BCCDC, and I/my dancer will then isolate as advised by the BC government and professional medical services.

TUITION CLASS FEES

For the September 2023 - June 2024 season, tuition is calculated on a thirty-seven (37) week term and is inclusive of all statutory holidays and breaks during the season. TUITION IS DUE IN FULL AT THE TIME OF REGISTRATION. However, you may opt into our payment plan (the "Payment Plan") where your tuition is divided into ten (10) equal monthly installments, starting September 2023 and ending June 2024. The monthly instalment amount will remain constant each month regardless of the number of classes in each month. If you choose the Payment Plan option, please note that in the comment section of the registration form or email us at udcstudios@gmail.com.. By opting in to the payment plan, the following applies:

Each monthly installment is due on or before the first day of each month. Installments will be paid by monthly E-transfers or credit cards which must be set up at the time of registration. Registration is not complete until this step is completed.

Accounts must be paid by the same method for all students on the same account.

New Families to UDC Studios (being the 1st year) will have to pay the tuition fees in 3 months instalments, September 2023 - December 2024, January 2024 - March 2024, April 2024 - June 2024.


Any accounts not paid in full on or before the first day of each month will be charged a $35 late fee and, if payment is not received by the 5th of the month, a $50 late fee will apply.

For any account past due more than thirty (30) days, the student(s) will be suspended from class participation until the account arrears are settled.

Accounts sixty (60) or more days past due may be sent to a collection agency and the entire remaining tuition balance will become immediately due and payable.

Please confirm at registration whether you are opting into the Payment Plan option.

COMPETITION FEES

Should your student be registered in a Company Program class, then your student will be participating in competition events this season. For each separate competition event, an entry fee will be charged per performance by the various dance competition companies and, as is common in the industry, each entry fee will be subject to an administration fee by UDC to cover the time and expense of organizing and registering in these events.

Competition fees must be paid in advance of the events and are typically invoiced in October and November with the scheduled due dates indicated on the invoices. In order to avoid late fees that may be imposed by the various competition companies, fees must be submitted prior to entry submission deadlines. Students with outstanding competition fees will not be eligible to compete in the scheduled competitions.

Competition fees are non-refundable and non-transferable even if your student decides not to participate for any reason, withdraws from a class, or is withdrawn from class as a result of one or more reasons stipulated in the CLASS WITHDRAWALS section.

Should an event be canceled by the competition company, any refunds or credits for entry fees will be as stipulated by the terms and conditions of the competition company. If a refund or credit is granted for entry fees, any associated UDC administration fees will not be refunded nor credited.

COSTUME FEES FOR COMPANY, RECREATIONAL CLASSES

All performance classes will require a costume for competition events and all performance events. Minimum deposits are required upon registration for a performance class.

The balance of costume fees will be invoiced when all the final costs are received from our seamstresses and costume supply companies. Costume fees must be paid in advance of competition events and all performance events. No costumes will be provided to the student until the costume fees are paid in full.

Company costume fees are estimated to range from $105 to $165 plus GST for each costume. A costume deposit of $85 is required for each registered performance class and payment will automatically be processed on September 1, 2022, via the payment method you have on account.

Parents are responsible for full payment of all costumes ordered, even if your student decides not to participate for any reason, withdraws from class, or is withdrawn from class because of one or more reasons stipulated in the CLASS WITHDRAWALS section. No refunds are provided for costumes.

YEAR-END PERFORMANCES

Year-End performance dates will be announced during the dance season. An admission fee will apply to each person attending the performances.

GENERAL GUIDELINES

Students, family members or friends should always refrain from negative comments and negative behaviour of any kind around any of our students, faculty or staff members. Such negative behaviour may result in your removal from the studio. Students flourish in a positive environment!

Do not discuss other students, family members or friends on the studio premises.

Please properly dispose of all trash or items that you may bring into the studio and ensure all food/trash wrappers are properly disposed. Please keep the studio as clean as possible. Be proud of your space.

Please keep noise to a minimum in the lobby, as higher noise levels are disruptive to the faculty and students.

UDC Studios takes no responsibility for lost or stolen items. Any found or returned items will be placed in the lost and found.

Theft of any property may result in immediate suspension or withdrawal from the studio.

Willful damage of studio property by you or your student may result in costs being recovered from you and immediate suspension or withdrawal of your student from the studio.

Students and parents are not allowed inside the dance studio classroom prior to the arrival of the dance instructor. Students are to remain in the common areas until classes begin.

UDC Studios is not responsible for unsupervised students.

PARENTAL GUIDELINES

The studio is not responsible for high and persistent absenteeism. No refunds or credits will be provided as a result.

Please be respectful of all that share our space and ensure your student cleans up their area after use.

If there are any changes to your personal information (phone number, address or email), please inform the administrative staff immediately.

Please walk students, especially younger students, into the building of the dance studio. This ensures that all students arrive to class safely.

Sometimes emergencies may arise where the studio may not be open. Please do not leave your student outside the studio unattended.

All students who are not with a faculty or staff member must be accompanied by a parent at all times inside or outside the studio.

Please stress the importance for your student to remain in the studio. No student should remain outside or leave the premises unless a parent or faculty member is present at that time. UDC Studios will not be responsible for any students that leave the premises.

Students may not leave during a class unless a written request has been provided by a parent/guardian.

Parents may not enter the dance studio classroom when classes are being taught, unless there is an emergency.

If you have questions or concerns, please address them with one of the directors directly. Please do not interrupt faculty during classes or between scheduled classes. If you wish to speak to a faculty member, please email your questions to us.

Please notify us in writing if your student needs to leave class early for any reason. This also includes the older students.

Please make arrangements to have your student picked up on time.

STUDENT GUIDELINES

No food, gum, and drinks are allowed in the dance studio classrooms at any time (bottled water is the only exception). All food and drinks must be kept in designated areas.

Always arrive to your class on time.

Attendance is important and any student missing too many classes may not be permitted to perform in competitions or dance recitals.

Please do not leave or walk out of the dance studio classroom without asking permission. The faculty member needs to know where you are at all times.

Your complete and undivided attention is expected during class.

Be courteous and respectful to all students, faculty and staff members at all times.

Students should place their name inside all clothing, shoes and dance bags.

Absolutely NO cell phones will be allowed in the classrooms during classes. If a student has a cell phone; it can be placed in his/her dance bag.

Do not wear street shoes on the dance floor.

All students must be in proper dress code while attending classes.

Relax, smile and have fun!

We will honour statutory holidays and no class instruction will be provided during statutory holidays. We may also close under local weather advisory.

UDC Studios does not provide make-up classes for classes falling on statutory holidays, classes cancelled due to local weather advisory conditions, or for classes cancelled due to circumstances beyond UDCs reasonable control. No tuition adjustments, credits, or refunds will be given as a result of these circumstances.

We are not responsible for classes missed due to sickness or vacation days. Make-up classes may be assigned or your student can attend a class of the same age and ability if a comparable class is available. No tuition adjustments, credits, or refunds will be provided for missed classes.

In the event that teachers are unavailable to teach their classes, these classes will be re-scheduled when possible.

Should UDC Studios be required to cancel in-studio classes due to circumstances beyond its reasonable control, UDC Studios will endeavour to offer classes through alternative formats such as on-line/virtual instruction in place of in-studio instruction. Students will be expected to attend classes via the alternative delivery formats offered to the students. These delivery formats will constitute valid substitute instruction and the Terms and Conditions of this Acceptance Agreement remains in full force and effect.

It is a requirement that all students have insurance coverage by their own family medical and insurance policies.

UDC Studios is not responsible for personal injury that may result from training in its dance studios. Further, the owners, directors and any affiliates associated with UDC Studios will be forever discharged, held harmless, and are NOT responsible for any injuries of any kind while participating at the studio, at any other event associated with the studio, or at any other location including the student's home. All students registered for classes are required to sign the "Assumption of Risk, Waiver, Release & Indemnity" Agreement attached to this document.

UDC Studios' primary form of communication is done electronically through e-mail. To ensure our e-mails arrive in your inbox, please mark any communication from UDC Studios as not spam or junk.

In the event UDC Studios is required to send an urgent communication studio wide, this will be done via social media and then followed up by e-mail.

PHOTOS AND VIDEOS

Permission is granted to UDC Studios to use images of students for learning and publicity purposes. These materials include, but are not limited to, photographs and videos of classes and performances, newsletters, other promotional materials and internet web pages.

UDC STUDIOS only accepts payments by automatic fund transfers or credit cards. Automatic fund transfers or credit cards must be set up at time of registration. The following dates indicate when applicable designated fees are due with payment automatically processing via the payment method on account.

* Parents must sign up for Autopay on the account and have a valid credit card on file.

* Costume deposits are due September 1, 2023.

* Competition fees are invoiced during the dance season in the months of October 2023 and November 2023 with the scheduled due dates.

* All Auto Pay through this site and Credit Card /Pay Pal payments will be charged on the first of every month for the dance season for registered classes.

* All Registration must be online due to limited office hours.

* Due to COVID we are not accepting In-Studio Payments.

*Any account that has 3 late payments or more you will agree to pay for the rest of tuition for the season in full.

There is a $50 fee for all automatic fund transfers and credit cards that are declined and cannot be processed. Late fees will also apply.

UDC Studios does not provide refunds for any Designated Fees. "Designated Fees" include, but are not limited to: registration fees, costume deposits, costume balances, exam fees, competition fees, administration fees, late fees, recital fees, or any other fees other than tuition.

The only exceptions to our no-refund policy for Designated Fees are as follows:

For competition entry fees where the third party organizer has provided corresponding refunds or credits to UDC Studios.

Designated fees are non-refundable and non-transferable even if your student decides not to participate for any reason, withdraws from class, or is withdrawn from class as a result of one or more reasons stipulated in the CLASS WITHDRAWALS section below.

UDC Studios does not provide refunds for tuition except as specifically described in the CLASS WITHDRAWALS section below.

At the sole and unfettered discretion of UDC Studios, overpayments and credits to an account may only be transferred to another dance season or to items needed during or at the end of the dance season.

This Agreement is between UDC Studios, located at #110-11960 Hammersmith Way, Richmond, BC V7A 5C9, telephone number (604)-277-4494 and you, the undersigned Parent or Legal Guardian of the Student(s) l am registering for. This Agreement is effective as of the date you sign this Agreement.

NOTICE- Despite any other language to the contrary, you may cancel this Agreement for any reason within 10 days of receiving a copy of this Agreement by giving written notice to UDC Studios. In this case, a full refund of all fees paid to date will be issued with the exception of the non-refundable registration fee of $35 plus GST paid at the time of registration. All other cancellation rights are outlined in the CLASS WITHDRAWALS section.

TERMS AND CONDITIONS OF ACCEPTANCE ARE STIPULATED IN THE FOLLOWING SECTIONS.

Assumption of Risk, Waiver, Release of Liability & Indemnity For Virtual or On-Line Classes

I, the undersigned, recognize and acknowledge that there are certain risks of physical and mental injury relating to participation in any and all activities in association with UDC Studio, and
I voluntarily agree to assume the full risk of any and all injuries, damages, death, or loss, regardless of severity, that I may sustain as a result of my participation.

Special Considerations for Virtual or On-Line Classes:
In addition to everything else in this agreement, in order to participate in online classes, I will ensure that the following conditions are met:

1. There will be a supervisory adult present in the home for my dancer during my participation in all virtual or on-line classes.
2. I will establish a reasonable physical space in my home which is free of tripping hazards, loose carpets, sharp objects, or any other impediment or a potential hazard when participating in the virtual or on-line classes.
3. My child will not participate in a virtual or online class if they are suffering from any injury or illness that may be aggravated or prolonged because of engaging in physical exercise of this type and nature.
4. I acknowledge that the flooring in my home may not be suitable for long periods of dance and I agree to monitor for any signs of joint or muscle fatigue and take breaks as appropriate.

I acknowledge and agree that UDC Studio, its owners, faculty members, employees, guest artists, independent contractors, assistants, volunteers, and/or students (collectively, “UDC Studio and its Agents”) shall not be liable for any injuries, damages, death, or loss however caused, which might be sustained by students, their families, guests, or others on or off UDC Studio’s premises and I agree to waive and relinquish all claims against UDC Studio and its Agents.

Further, I do hereby fully release and forever discharge UDC Studio and its Agents from any and all claims for injuries, damages, death, or loss that I may have or which may accrue to me arising out of or connected with my participation in any in-person, virtual or on-line classes or other activities on or off UDC Studio’s premises and whether or not such claims arise from the negligence, omission, or willful action of UDC Studio or its Agents.

The undersigned parent, legal guardian, or student of the age of majority agrees to hold harmless and indemnify UDC Studio and its Agents from any and all liability, claims, demands, or causes of action whatsoever and howsoever arising out of or in connection with participation in any in-person classes, virtual or on-line classes, or other activities conducted by or in association with UDC Studio.

Permission is granted to UDC Studio to use images of students for learning and publicity purposes. These materials include, but are not limited to, photographs and videos of classes and performances, newsletters, other promotional materials, and internet web pages.

This agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns and representatives, in the event of my death or incapacity.

By signing this waiver for my child or children, I certify that I am the parent or legal guardian and I have the right to provide this assumption of risk, waiver, release, and indemnity.

I have read, and fully understand and agree to be bound by the above terms.
