Welcome and we are looking forward to dancing with you soon! After you complete the online registration, please visit our front desk to sign a medical/photography release form, sign our parent/dancer policy, and set-up your payment arrangements. Official registration in all classes is confirmed after the first class is attended by your dancer. New students, we want to make sure we have the perfect class for each dancer! Please consult with the office staff before attending class for placement approval and availability. We are happy to evaluate your level and find the right fit for you. Returning students have been evaluated throughout the last season and summer classes. Placement and scheduling is customized for each student based on their needs. Changes are made only upon recommendation of the teaching faculty. Our curriculum is designed for dancers to stay in each level for 1 –3 years. Students will move to the next level once they have met the requirements of their current level. You must be 18 or older to register for a new account. I understand that there are risks of physical injury associated with, arising out of and inherent to the
activity of dancing, vocals, acrobatics, acting and modeling. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all rights and/or causes of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Supreme Perfection Studios, its officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities (hereinafter collectively referred to as "SPS").
I hereby agree to release SPS and hold SPS harmless of all liability, and herby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dancing on behalf of the participant. I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
I also give SPS my permission to use my child's picture or video on any form of advertisement for SPS or a SPS affiliated event. If I am a minor, my parent and/or legal guardian has also signed this document releasing SPS from and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity. The above named participant has my permission to participate in all SPS events. I warrant the above information is complete and correct. I hereby authorize SPS or their agent to act on my behalf to provide emergency medical treatments. I further release SPS of all liabilities associated with my child attendance in a SPS event.
